Role Summary
As a Hybrid Telehealth Physical Therapist, your primary responsibility will be to develop and implement rehabilitation programs aimed at restoring and enhancing the physical capabilities of our patients. You will create individualized treatment plans that align with the specific objectives of each patient, facilitating their return to work and daily activities with confidence. This role involves leveraging telecommunication technologies to provide remote physical therapy services, supported by an on-site Physical Therapy Assistant for data collection and patient support. The goal is to ensure all tasks are completed with precision, quality, and efficiency.
Key Responsibilities
- Evaluate the rehabilitative needs of patients and formulate suitable treatment strategies.
- Continuously update and refine treatment plans based on patient progress.
- Employ clinical judgment in the assessment, planning, execution, and monitoring of patient outcomes.
- Educate and guide patients on exercises, home care routines, and overall wellness.
- Maintain clear communication with patients and referring healthcare providers regarding treatment plans and progress.
- Foster a respectful and dignified environment for patients, families, and colleagues.
- Perform additional job-related tasks as required.
Qualifications
- Doctorate or Graduate Degree in Physical Therapy from an accredited institution.
- Active Physical Therapy license in California is mandatory.
- Strong commitment to patient care and success.
- Proficient in Microsoft Office and patient management systems.
- Familiarity with physical therapy administrative processes and protocols.
- Excellent verbal and written communication abilities.
- Outpatient experience is preferred; however, new graduates are encouraged to apply.
