SUBSTANCE USE DISORDER CARE COORDINATOR-(CCBHC)REPORTS TO: PROGRAM DIRECTOR
MISSION STATEMENT: Center Point, Inc.'s (CPINC) mission is to provide comprehensive social, educational, vocational, medical, psychological, housing, and rehabilitation services to combat social problems such as substance abuse, poverty, unemployment, and homelessness. CPINC offers rehabilitation and treatment services that interrupt the abusive cycles of psychological, social, and economic dislocation by providing critical training and support so that individuals and families can claim self-worth and dignity.
FUNCTION OF THE POSITION:CPINC employees are required to model the agency's values as outlined in the Personnel Policies Manual. CPINC employees are required to maintain the Center Point's philosophy and quality standards. It is the responsibility of every employee to adhere to all federal, state, and local authorities' rules and regulations. This position is a multidisciplinary team member, ensuring that the coordinated systems of care are responsive to the assessed needs of clients in a timely and comprehensive manner. The SUD Care Coordinator ensures the integration of substance use and mental health disorder treatment and primary care services in a culturally competent, trauma-informed, ethical, legal, and moral manner within a safe and therapeutic environment consistent with applicable regulatory and accreditation standards. The SUD Care Coordinator is an integral part of the clinical team and participates in delivering addiction treatment services to clients. The services include providing individual and group counseling and outreach to the community. The SUD Care Coordinator also performs other duties to ensure overall program and contract compliance and quality assurance within Center Point programs.
EDUCATION AND WORK EXPERIENCE REQUIREMENT:
  • At least three years as a Certified Substance Use Disorder Counselor or one year as an Associate registered through BBS or Board of Psychology.
  • Knowledge of drug addiction/substance use and co-occurring disorders, abuse patterns, treatment methodologies, and medication-assisted treatment.
  • Experience utilizing ASAM and other assessment tools to recommend treatment placement for clients.
  • Ability to provide outreach to the community and make client referrals to providers as needed.

Preferred Qualifications
  • Experience with client data collection, project reporting, and analysis.
  • Experience in providing individual and group SUD treatment in an Outpatient setting.
  • At least two years of work experience caring for individuals with substance use disorders.

QUALIFICATIONS, KNOWLEDGE & SKILLS:
  • Have a valid California Driver's license and maintain a satisfactory driving record.
  • Strong use of the English language in verbal and written communication.
  • Have a "can do" attitude and be a team player.
  • Ability to work in a fast-paced environment and maintain poise under pressure.

COMPUTER RELATED SKILLS:
  • Ability uses new software programs with basic training.
  • Familiar with Electronic Medical Record systems and the ability to enter client information in such systems.
  • Ability operates a desktop or laptop computer.
  • Proficiency with Microsoft Office Suite, Web site navigation, and desktop software programs.

ESSENTIAL DUTIES AND RESPONSIBILITIES:CPINC conducts yearly job performance reviews to ascertain compliance with established policies and procedures. The employee must minimally maintain the following duties and responsibilities:
Primary Duties:
  • Evaluates clients for program eligibility, conducts necessary screenings, and makes referrals for further assessments for admission into the SUD system of care.
  • Participates as a member of the utilization review committee to determine treatment plans, care coordination plans, treatment plan reviews, progress notes, and other required documentation to ensure compliance with agency, federal, state, and local government standards. Assures compliance with all Drug Medi-Cal and Department of Health Care Services Regulations.
  • Provides primary care coordination services to clients, family members, and other stakeholders as relevant. Ensures the efficacy of referrals and linkages to all levels of care provided directly and by referral and acts as the primary liaison between CPI and other providers as permitted by HIPAA and 42 CFR, Part.
  • Participates in the collection and compilation of CCBHC Intake information as well as update of client data for National Outcome Measures.
  • Community outreach, education, and engagement to promote relationships with stakeholders, other agencies, and advisory groups. Participate in community meetings, parole and probation re-entry committees, and other forums to advocate for clients.
  • Provides staff training and development regarding evidence-based practices and curriculum related to care coordination and linkages to care.
  • Interacts with diverse clients and staff in a culturally competent manner
  • Maintains professional, courteous interactions with clients, parolees, parole and probation officers, other agencies, the public, board members, and senior management.
  • Participates in multidisciplinary case conferences, weekly staff meetings, and clinical reviews.

PHYSICAL REQUIREMENTS:
  • Must be in good physical and mental health and capable of meeting the program.
  • Ability: sit, walk, stand, bend, squat, climb, kneel, and twist intermittently or continuously.
  • Ability grasp, push and pull objects such as files and file cabinet drawers and reach overhead.
  • Ability lift and carry up to 25 lbs.
