Job Summary: The Community Care Liaison will be responsible for building and maintaining relationships with clients, their families, caregivers, and community partners to ensure the delivery of high-quality care services. In this role, you will serve as a bridge between our agency and the community, focusing on sustaining our marketing and outreach efforts to promote our services along with maintaining strong relationships with our existing clients. Although you will have the opportunity to build relationships with all of our referral sources (IDOA, Assisted/Independent living facilities and private clients) this role will place an extra emphasis on building a relationship with Veterans Affairs and other VA organizations. The ideal candidate will possess strong sales experience, excellent communication skills, empathy, and a passion for serving others.
Qualifications/Educational Requirements:
- A bachelor’s degree in social work, healthcare administration, or a related field is preferred but not required.
- Proficiency in both English and Spanish is preferred.
- Strong knowledge of non-medical homecare regulations, standards, and best practices.
- Prior experience working with Veteran Affairs or in a similar role, with a strong understanding of VA programs and services is preferred.
- Any certifications related to veteran services or care coordination are a plus.
- Excellent communication skills, with the ability to motivate and engage a team of personal care attendants.
- Compassionate and empathetic approach when working with clients and their families.
- Proficient in computer skills, including word processing, email, and electronic health record systems.
- Strong organizational and time-management skills, with the ability to prioritize tasks and meet deadlines.
- Ability to handle sensitive and confidential information with professionalism and integrity.
- Veterans or individuals with military experience are strongly encouraged to apply.
Responsibilities:
- Community Relationship Building - Build and maintain strong relationships with community organizations, Veteran Affairs, healthcare providers, senior centers, retirement communities, clients/families and other relevant entities. Attend community events, health fairs, and networking opportunities to promote the agency's services and establish partnerships that can lead to client referrals.
- Proactive Outreach - Reach out to potential clients, their families, and caregivers to educate them about the benefits of homecare services. Utilize various communication channels, such as phone calls, emails, and in-person meetings, to provide information, answer inquiries, and address concerns. Maintain a professional and empathetic approach to foster trust and confidence in the agency.
- Collaboration - Collaborate with internal teams to coordinate the details of the outreach/marketing events, including scheduling and logistics as needed.
- Participation and Support - Assist in community outreach activities to promote the agency's services and build relationships with local organizations and healthcare providers.
- Documentation - Maintain accurate and up-to-date documentation of client interactions, care plans, and service delivery activities along with assisting in other administrative duties as needed. Adhere to all relevant policies, procedures, and regulations governing the provision of homecare services.
- Referral Program Development - Establish and manage a referral program to incentivize satisfied clients, healthcare professionals, and community partners to refer potential clients to the agency. Develop referral materials, track referral sources, and provide recognition or rewards as appropriate.
- Data Analysis and Reporting - Assist the marketing team in Tracking and analyzing our marketing efforts, referral sources, and client acquisition metrics. Prepare regular reports for the management team, highlighting key performance indicators, trends, and recommendations for improvement. Use data insights to refine marketing strategies and tactics.
- Compliance and Ethics - Adhere to all relevant laws, regulations, and ethical guidelines related to marketing and client engagement in the healthcare industry. Maintain confidentiality and handle sensitive information with the utmost care and professionalism.
