SUMMARY:
  • The Independent Living Coordinator works to uphold excellent consumer service standards by managing resources and providing hands-on support and training for Personal Care Attendants.
  • All activities must reflect the Independent Living Philosophy.
  • All duties must be carried out in accordance with the organizational policies and applicable laws. All activities must reflect the Independent Living Philosophy.
  • The IL Coordinator will abide by the laws of the Commonwealth of Pennsylvania.
  • The IL Coordinator will comply with the policies and procedures established by DON SERVICES in the Employee Handbook.
  • The IL Coordinator will perform services in the consumer control manner whenever they are related to the consumer’s service plan.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: IL COORDINATOR RESPONSIBILITIES:
  • Schedule on-call and pre-scheduled attendants for consumers.
  • Arrange for backup attendant care.
  • Fill shifts when backup care is unavailable. 
  • Inform the supervisor of problems relating to employed attendants.
  • Record reasons for attendant call-offs or refusing work.
  • Verify hours worked by attendants 
  • Responsible for pre-employment screening of potential attendants prior to meeting the consumer.
  • Maintain weekly communication and documentation to meet consumer needs.
  • Visit consumers on a quarterly basis to maintain proper documentation and regulate optimum care. 
  • Arrange and provide training to attendants on duties listed in individual service plans, independent living philosophy, and paperwork related to job duties. 
  • Monitor consumer care in accordance with the Office of Long-Term Living by ensuring consistency and accuracy in services provided in terms of type, scope, amount, duration, and frequency. These are defined as follows: 
    • Type: The provider is qualified and enrolled with the Department to provide the waiver service. 
    • Scope: The specific services to be delivered must match the service definition. 

Amount:
The number of units ordered and documented on the service plan is delivered to the participant with reasonable documentation to show why services were not provided.
Such reasonable documentation is as follows:
  • Consumer refuses services. 
  • Consumers are ill and services cannot be delivered.
  • Family members delivered services as unpaid caregivers.

Duration:
  • Services must be provided in the amount of time scheduled.
  • Documentation must be kept showing that services were or were not provided. If services are not provided, documentation must be kept showing why. 

Frequency:
  • Services are provided the scheduled number of times, for example, once per month, once per week, etc. 
  • Other duties as assigned.

AUTHORITY: 
  • The employee may proceed with duties and responsibilities within the following limitations:
  • The IL Coordinator must obtain approval on all activities within the service plan from the consumer and all activities outside of the service plan from the consumer.

QUALIFICATION/REQUIREMENTS: 
  • To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

EDUCATION and/or EXPERIENCE: 
  • GED or high school diploma; or up to one-month related experience or training; or equivalent combination of education and experience. 
  • References stating that a person can competently perform this work will be required.

LANGUAGE SKILLS: 
  • Ability to read a limited number of two and three-syllable words and to recognize similarities and differences between words and between series of numbers. 
  • Ability to print and speak simple sentences.

MATHEMATICAL SKILLS: 
  • Ability to add and subtract two-digit numbers and to multiply and divide with 10’s and 100’s.
  • Ability to perform these operations using units of American money and weight measurement, volume, and distance.

REASONING ABILITY:
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Ability to deal with problems involving several concrete variables in standardized situations.

CERTIFICATES, LICENSES, REGISTRATION:
  • IL Coordinator must be able to demonstrate a problem-free record under PA ACT 33 and 34.

OTHER SKILLS AND ABILITIES:
  • Strong people-oriented skills must be demonstrated.
  • Familiarity with computer software systems (HHAX, Excel, and Google Docs).
