Job Description:
As a Hospice RN, you'll embark on a unique journey, delivering exceptional care to patients in the comfort of their own homes across different neighborhoods and communities. This role combines your nursing expertise with a sense of adventure, allowing you to make a difference while exploring new horizons
Responsibilities:
- Conduct comprehensive assessments and develop individualized care plans for patients receiving home health services.
- Provide skilled nursing care, including wound care, medication management, and patient education, to promote optimal health and independence.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to coordinate holistic care and support services for patients.
- Monitor patient progress, evaluate outcomes, and adjust care plans as needed to ensure continuity and quality of care.
- Educate patients and families on disease management, medication compliance, and self-care techniques to empower them to take control of their health.
- Maintain accurate documentation of patient encounters, assessments, and interventions in compliance with regulatory standards and agency policies.
- Active RN license with a minimum of 2 of clinical experience in home health, community nursing, or a related field.
- Excellent communication skills and the ability to establish rapport with patients and families from diverse backgrounds.
- A love for travel and exploration, with the flexibility to work in various neighborhoods and communities within 30 minutes.
- Compassion, empathy, and a genuine commitment to providing person-centered care with dignity and respect.
- Strong organizational skills and the ability to adapt to changing environments and priorities.
