Responsibilities:
- Provide comprehensive care to hospice patients in their homes, including assessments, care planning, and medication administration.
- Collaborate with the care team to ensure seamless care coordination and communication.
- Monitor patient progress, update medical records accurately, and provide support to patients and their families during difficult times.
- Exercise sound judgment, supervise others effectively, and handle crises outside a hospital setting.
Requirements:
- Current RN license in the state.
- Completed a Hospice training program.
- At least 2 years of nursing experience, including recent patient contact.
- Strong communication and interpersonal skills, with adaptability to diverse cultural and religious backgrounds.
