General: Magnolia Hospice provides care to terminally ill patients in their home. Settings include private homes, nursing facilities, assisted living, memory care, senior apartments and other settings. 

Duties include:

  • Assessing and providing hands on care
  • Admissions after hours
  • After hours discharges including death pronouncement
  • Communicating with the interdisciplinary team
  • Documenting necessary information in the electronic healthcare record
  • Collaborating with the hospice physician
  • Coordinating necessary ancillary services such as DME, lab, xray, transportation
  • Providing services collaboratively with other healthcare providers such as in nursing facilities, hospitals and home health
  • Oversight and direction of unlicensed assistive personnel
  • Education and support for family, caregivers and facility staff

Opportunities are available for full or part time positions.

Requirements:

  • Position requires dependable transportation without restriction.

Other requirements include:

  • Unencumbered licensure in Texas. License in compact state will be considered.
  • Current healthcare provider CPR
  • Ability to perform essential functions of job with or without reasonable accommodation
  • Must pass criminal history check and other required background checks
  • Drug and alcohol screening
  • One year of registered nursing experience. Two years with hospice preferred
  • Bilingual English and Spanish or other languages a plus
  • Homecare Homebase experience a plus
