We are seeking a highly skilled and compassionate Hospice Team Assistant to join our team at Austin TX HOS. As a key member of our healthcare team, you will provide exceptional support to our patients and their families, ensuring their comfort and well-being throughout their journey.
Responsibilities:
- Provide administrative support to our healthcare team, including scheduling appointments, managing patient records, and coordinating communication with patients and families.
- Assist with patient care tasks, such as vital sign monitoring, medication administration, and wound care.
- Collaborate with our interdisciplinary team to develop and implement patient care plans, ensuring seamless coordination of care.
- Communicate effectively with patients, families, and healthcare providers to ensure accurate and timely information exchange.
- Participate in quality improvement initiatives, identifying areas for improvement and contributing to process enhancements.
Requirements:
- High school diploma or equivalent required; associate's or bachelor's degree in a healthcare-related field preferred.

- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
