Job Summary:

A Community Health Worker (CHW) is the link between patients and health and social services. CHWs are trusted members of the community that help to facilitate access to services, improve health outcomes, reduce barriers to receiving care, and create relationships that foster positive lifestyle changes.

Community Health Workers are frontline public health workers. The are trusted members of the community that have a close understanding of the communities they serve.

A Community Health Worker builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, individual and community education, informal counseling, social support and advocacy.

Key Functions and responsibilities:

1. Provide individual and group education on mental health and substance use and related public health concerns to NACC clients and the Native American community

2. Provide information and referral to NACC and other partnering programs to meet the comprehensive needs of clients

3. Identify, create and implement client education materials to share in groups and at community events

4. Work with clinical staff to develop program protocol and workflow implementation.

5. Explore patient resources that support success create resource tools to be used in the clinic and community

6. Respond to and fulfil request for community public health education

7. Provide harm reduction client education and educational materials

8. Collect and maintain up to date data related to grant outcomes and report regularly

9. Active member of harm reduction services team

Knowledge, skills and abilities:

1. Computer skills including web browsing, excel, and EMR software

2. Patient and community education background

3. Excellent people skills; ability to work independently and as a team member

4. Understanding of HIPAA regulations, respect for patient confidentiality

5. Efficiency and attention to detail

Training and experience: 1. Must have a valid certificate from the Minnesota State Colleges and Universities (MnSCU) demonstrating completion of an approved community health worker

curriculum or completion of the IHS National CHR Program training and education program.

2. Two years of direct care experience working with mental health and substance use disorders

3. Experience in the Native American community preferred

4. Flexibility to work nights and weekends, if needed
