POSITION SUMMARY:

This position is responsible for working directly with individuals experiencing homelessness who have a serious mental illness and/or co-occurring substance use disorder. This requires the ability to work in non-traditional settings and unstructured environments and meeting individuals experiencing homelessness where they are, including in the woods, under bridges, and in abandoned houses. Must be organized and able to prioritize tasks quickly.


REPORTS TO:

Lead Outreach Worker.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Work with referral sources and community partners to identify applicants through team meetings, outreach, and referrals.

• Complete interviews with individuals to gather medical records and information necessary to complete SSI/SSDI applications.

• Write SOAR Medical Summary Reports for individual applications.

• Accompany individuals to appointments at the Social Security Administration (SSA).

• Coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for the application.

• Coordinate case management services with partners, provide direct case management services individuals when needed.

• Perform targeted street outreach in a variety of settings during non-traditional hours and in collaboration with partner agencies.

• Travel for the purpose of meeting with clients, training, collaborating with stakeholders, or off-site personnel/management.

• Attend local provider meetings to include but not limited to local ECAN, ECAN Steering and Operations committee meetings and local Community Care Team meetings.

• Participate in the ECAN intake, assessment, and triage process to include 211 assessments.

• Connect identified individuals to housing, behavioral healthcare, medical care, food and clothing assistance, transportation, recovery support and case management.

• Attend any trainings/certifications as required by the state.

• Determine eligibility for PATH utilizing the VI-SPDAT assessment; provide case as indicated.

• Utilize HMIS and DDAP systems to document admission, discharge, activities, and to access quality assurance reports.

• Complete all documentation in a timely manner in accordance with funder and/or regulatory requirements.

• Adhere to the Intended Use Plan (IUP) and ensure goals are met for the application year.

• Assist the team with administrative tasks as needed.

• Other duties as assigned.


MINIMUM REQUIREMENTS:

  • Bachelor’s degree in a related field preferred, or certification as a Peer Specialist. Prior experience may be considered in lieu of education.
  • Minimum of 2 years working with individuals experiencing homelessness who have a serious mental illness and/or co-occurring substance use disorder.
  • Ability to work in the community and meet individuals experiencing homelessness where they are, including in the woods, under bridges, and in abandoned houses.
  • An ability to work in non-traditional settings and unstructured environments.
  • A valid state driver’s license with a clean driving record.
  • Ability to prioritize multiple tasks and meet frequent deadlines.
  • Superior organizational skills and attention to detail and ability to prioritize tasks quickly
  • Computer experience, including troubleshooting and problem solving and proficiency in Microsoft Office 365 applications and database management.
  • Excellent verbal and written communication skills; ability to analyze extensive data and create written reports with accuracy and brevity.
  • Ability to maintain professional boundaries and engagement skills with a challenging population and in non-traditional work conditions.
  • Physical requirements include: Frequent travel by foot, car, or other means appropriate to making contact with population; Ability to adjust to the environment of the target population including visits to encampments, shelters, and personal living environments when needed; Sitting for extended periods of time.

PREFERRED QUALIFICATIONS:

  • SOAR training.
  • Prior experience with providing outreach and successful engagement to a diverse population, includes working with clients, developing trust, and conducting interviews in non-traditional settings and unstructured environments.
  • Evidence of ongoing training and education in related areas such as mental illness, substance abuse, and/or homelessness.
  • Lived experience of homelessness and/or mental health disorder.
  • Familiarity with Social Security Administration’s Listings of Impairments.
  • Cursory knowledge of medical and psychiatric terminology and ability to write using same.
