POSITION SUMMARY:

This position is responsible for providing transitional case management and support services for individuals in sober living environment who are re-entering the community from incarceration. Duties include admission and discharge planning, individual case management, and facilitate psychoeducational groups.


REPORTS TO:

Residential Director.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Monitor and track client progress, including resident adherence to individualized re-entry plan.
  • Provide ongoing support, coaching, and encouragement during transitional and throughout stay, particularly in available resources within the local recovery community.
  • Identify needs and assist individual in gaining access to financial benefits and other supportive services including but not limited to medical, social, housing, family planning and reunification, and educational services.
  • Provide linkage to mental health and substance use treatment services, including a wide range of treatment modalities and recovery communities.
  • Assist with obtaining reliable means of transportation, including use of public transportation.
  • Provide coaching and linkage to employment services.
  • Assist with use and tracking of contracted resources, including rental assistance, gift cards, bus passes.
  • Provide one hour per week of in person case management based on individual recovery plans.
  • Facilitate a minimum of two psychoeducational groups per week.
  • Perform other duties as assigned.


MINIMUM REQUIREMENTS:

  • High School diploma or GED equivalent.
  • Certificate of completion from a recognized peer/recovery coach program required.
  • Willingness to disclose status of recovery, with a minimum of two years of recovery from Substance Use/Mental Health Disorders.
  • Minimum of 2 years’ experience in related field.
  • Valid driver’s license in good standing.
  • Prior knowledge of mental health and substance use strongly preferred.
  • Lived experience with mental health and/or substance use disorders strongly preferred.
  • Ability to learn new systems and programs quickly.
  • Excellent verbal and written communication skills.
  • Excellent customer service skills.
  • Ability to develop clear and professional documentation.
  • Willingness and ability to establish and maintain healthy working relationships.
