POSITION SUMMARY:

This position is responsible for providing community outreach to unsheltered individuals and assist with securing stable housing and related needs. Duties include connection to care, assistance accessing benefits, and general case management support. This is a mobile position that requires non-traditional hours and collaboration with partner agencies.


REPORTS TO:

Program Lead.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Perform street outreach to identify and locate unsheltered individuals, including homeless shelters, partner agencies, community settings, and homeless encampments.
  • Assist with securing safe, secure, stable housing and related basic needs.
  • Develop strategies to increase access to services for unsheltered individuals.
  • Work collaboratively with individuals, their family, community partners, and agency professionals.
  • Collaborate with other service and treatment providers in the region.
  • Attend community events to raise awareness and provide education to reduce stigma of mental health and substance use services.
  • Complete all required documentation in a professional and timely fashion.
  • Perform other duties as assigned.

MINIMUM REQUIREMENTS:

  • Bachelor’s degree preferred. Experience may be considered in lieu of education.
  • 1 year experience in human services preferred.
  • Valid driver’s license and ability to drive agency vehicle required.
  • Ability to work non-traditional hours and in community setting.
  • Bilingual Spanish preferred but not required.
  • Prior knowledge of mental health, substance use, and recovery-based supports strongly preferred.
  • Understanding of trauma- and gender-informed care and services.
  • Ability to develop clear and professional documentation.
  • Willingness and ability to establish and maintain healthy working relationships.
