Family Life Coach

Position Summary
The Family Life Coach provides comprehensive case management services for 15-18 families experiencing instability. The Life Coach will offer education and support services to parents, using strength-based practices. Responsibilities include conducting initial and follow-up assessments; working cooperatively with other agencies to link families to financial services and community resources that meet the parents’ expressed needs; and providing direct services and accurate data entry to track and record contacts. Flexible schedule required as this position will work occasional evenings and weekends as needed.
Essential Duties
- Contact referred families for program enrollment, offering case management and program services.
- Engage and interview families for program enrollment and provide needs assessment using the tools determined by grant funding. Offer strength-based case management services, and follow-up contacts to assess goal attainment and need for further referrals. Manage a case load of 15-18 families at a time.
- Conduct home visits or visits at other locations convenient for family (weekly, semi-monthly, monthly) as determined by family to provide needs assessment, establish client rapport, set family-centered and strengths-based service plan, and provide direct services and follow-up to assigned families.
- Follow-up contacts to assess goal attainment and need for further referrals and providing resources that may not be available in the community such as, financial planning, parenting skills and communication skills.
- Develop effective case plans with families regarding parenting skills, family problems, economic stressors, parent/child relationships and community connectedness. Work toward keeping children safe in their home and stable in their school setting. Promote protective factors to families to ensure child safety.
- Report to supervisor including information on number of referrals received, home visits completed, and allocation of funds.
- Maintain accurate reports and database of client demographics and services provided for clients. Use forms to collect demographic and program information, for appropriate documentation of case plans, recommendations, contacts, assessments, family engagement activities, etc.
- Assist with tracking expenditures/revenues to comply with program budget.
- Integrate a 2Generation, whole family approach guided by family voice.
- Participate in ongoing trainings and be responsible for following Quality Standards for Family Strengthening & Support and Trauma-Informed Care.
- Provide advocacy to entities involved with family, based on family needs, including but not limited to welfare and public benefit agencies, landlords, and educational entities.
- Participate regularly with Family Connections Life Coaching team in ongoing case conferencing and community of practice.
- Participate regularly with appropriate community meetings
- Position requires regular travel between sites – Catholic Charities, home visits and participating organizations/schools
Competencies
- Case management skills and ability to define, collaborate, and facilitate attainment of stated goals.
- Good computer skills, including word processing and ability and to learn different databases.
- Knowledge of family dynamics, systems theory, domestic violence, and social work theory/ practice.
- Ability to engage, foster, and maintain effective working relationships with a broad variety families.
- Knowledge of child development and 2Gen strength-based family support practices and ability to use these practices when working with families and co-workers.
- Demonstrated proficiency in problem solving/planning, mediation, and conflict resolution.
- Knowledgeable and understanding of Trauma Informed Care principles/practices and Motivational Interviewing, and/or will take initiative toward self-directed learning of these.
- Utilize a Family Development approach to strengthen families and build support systems.
- Knowledge of social issues such as poverty, gender bias, domestic violence, alcohol and other drug abuse, child maltreatment prevention, cultural diversity, etc.
- Knowledge of community resources and systems.
- Ability to demonstrate commitment to developing community members' potential, prosperity and protection with demonstrated understanding of cultural awareness.
- Able to demonstrate attention to detail, following directions, prioritizing projects, and interpreting policies and procedures to ensure compliance.
Work Environment
- A majority of the assigned duties are performed in an office environment.

Requirements
- Bilingual English/Spanish strongly preferred
- Transportation and valid driver’s license with satisfactory driving record required. Local travel between partner schools, home visits, and Catholic Charities offices will be required.

- Occasional evenings and weekends are required to accommodate family schedules, meetings, and special events.
Education/Experience
- Associates degree in Social Work, Human Services, Psychology or related field and two to four years of full-time experience working with direct client/family interaction.
- Bachelor’s degree in Human Services, Social Work, Psychology or related field preferred along with one-year full-time experience working with direct client/family interaction.
- Knowledge of Human Services agencies in the area.
Compensation
- The salary range for this position is $18.51 - $20.00 / hour. Salary is determined based on the applicant’s incoming skills and experience and the budget at time of hiring.
