Job DescriptionJob DescriptionThe Medical Records Clerk is responsible for processing patient medical records including all aspects of inputting/ exporting information efficiently and accurately.

Essential Duties and Responsibilities:
  • Processes requests and authorizations for release of medical information.
  • Reviews all documentation for completeness and routes incomplete documents to the appropriate individual for correction prior to placing/scanning in the medical records.
  • Provides prompt customer service for walk in and phone call medical records requests from patients and healthcare providers.
  • Schedules third party chart audits and provides auditor feedback to supervisor for appropriate correction of any standards that have not been met.
  • Scans/indexes individual documents into electronic health record.
  • Processes subpoena or court ordered requests for information in accordance with the law.
  • Sorts and distributes all incoming faxes and submits any lab/study/pathology report and specialist consult reports to appropriate provider for review.
  • Facilitates death certificate processing and physician sign off.
