Position Overview

As a RN Clinical Assessor , you will conduct onsite assessments to facilitate appropriate placement and care for local populations in Coos County, New Hampshire. Your role involves providing essential evaluations and recommendations to enhance patient well-being and quality of life. These assessments, primarily conducted in nursing homes or hospital settings, play a crucial role in determining the level and scope of care required for each patient. Your assessments will be instrumental in securing better health outcomes, providing critical support that aligns with the specific needs of each individual.

As a trusted expert, you will have the autonomy to evaluate their care needs, determinations and recommendations for their ongoing level of care, helping to improve patient's well-being and enhance their quality of life. This is a PRN hybrid role that combines working from your home office with traveling to patients within your local areas. Must be open to covering the Coos County area. All required travel expenses, including mileage will be fully reimbursed.

Job Responsibilities

  • Conduct thorough clinical assessments of patients, reviewing their medical records, consulting with collateral sources, and analyzing available data to ensure accurate evaluations, recommendations, and determinations.
  • Oversee and enforce adherence to standards of practice in determining the appropriate Level of Care.
  • Collaborate with office staff to facilitate efficient workflow processes.
  • Engage with healthcare professionals, patients, their families, and other stakeholders to provide comprehensive support and care.
  • Perform timely annual reassessments to identify potential areas for improvement and necessary adjustments in care plans.
  • Apply independent judgment, clinical expertise, effective communication, problem-solving, and conflict resolution skills to achieve optimal outcomes for members, providers, and clients.
  • Formulate precise level of care recommendations based on comprehensive clinical evaluations.
  • Ensure that daily operations comply with contractual obligations and regulatory standards.
  • Foster positive and professional relationships, serving as a liaison with internal and external parties to enhance cooperation and streamline the review process.
  • Utilize state and internal electronic systems for completing and tracking tasks efficiently.
  • Note that this overview is not exhaustive. Responsibilities may be expanded to include additional duties as necessary, providing opportunities for professional advancement and skill enhancement.

Required Qualifications, Skills, Knowledge, and Experience

  • Completion of a nursing program from an accredited institution, earning either a Diploma, Associate Degree, or Bachelor’s Degree in Nursing. This education provides a strong foundation in the nursing field.
  • Active, unrestricted Registered Nurse (RN) license in the State of New Hampshire.
  • Advanced clinical assessment and critical thinking skills, demonstrating the ability to make informed decisions and deliver high-quality care.
  • Strong verbal and written communication skills, adept at clearly and effectively conveying complex medical information to diverse audiences across various platforms.
  • Proven ability to work effectively within a team, enhancing a collaborative and supportive workplace culture.
  • Exhibit flexibility, strong organizational prowess and skilled in managing multiple priorities with efficiency and accuracy.
  • Proficiency in Microsoft Excel, Word, and Outlook, with competent use of computer equipment and web-based software to ensure smooth workflow.
  • Strong communication capabilities with individuals and groups in both spoken, written, and electronic formats, improving engagement and understanding.
  • Meticulous attention to detail with a proven ability to prioritize tasks effectively and execute them with precision and timeliness.

Compensation

The compensation for this role is $34.00 to $35.00 per hour. Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
