As a Hospice RN Case Manager, you will play a critical role in providing compassionate care and support to patients who are in the final stages of life. Working within the hospice team, you will be responsible for coordinating and managing the overall care plan for patients and their families. Your primary focus will be on ensuring that patients' physical, emotional, and spiritual needs are met while promoting comfort and dignity during their end-of-life journey.

Compensation:

$43 - $48 hourly

Responsibilities:

Patient Assessment and Care Planning:

  • Conduct comprehensive assessments of patient's medical conditions, symptoms, and pain management needs.
  • Develop individualized care plans in collaboration with the interdisciplinary hospice team and the patient's family.
  • Regularly review and update care plans to address changing needs and ensure the highest quality of care.

Nursing Care and Treatment:

  • Administer skilled nursing care to patients, including wound care, medication management, and symptom control.
  • Monitor patients' vital signs, reactions to treatments, and overall well-being.
  • Collaborate with other healthcare professionals, such as physicians and palliative care specialists, to enhance patient comfort and pain management.

Emotional and Spiritual Support:

  • Provide emotional support and counseling to patients and their families during the end-of-life process.
  • Respect and accommodate patients' spiritual beliefs and cultural practices, promoting a sense of peace and acceptance.

Coordination of Services:

  • Facilitate regular interdisciplinary team meetings to discuss patient care plans and address any issues or concerns.
  • Coordinate the services provided by other members of the hospice team, including social workers, chaplains, and home health aides.
  • Liaise with external healthcare providers and community resources to ensure comprehensive support for patients and their families.

Patient and Family Education:

  • Educate patients and their families about the hospice philosophy, the end-of-life process, and available resources.
  • Provide guidance on pain management techniques and strategies for coping with emotional challenges.

Documentation and Compliance:

  • Maintain accurate and up-to-date patient records, documenting all care provided and communicating with patients and families.
  • Ensure compliance with all relevant regulations, policies, and procedures related to hospice care.

Bereavement Support:

  • Provide support to the family after the patient's passing, assisting with bereavement counseling and resources.
Qualifications:
  • Active Registered Nurse (RN) license in the state of practice.
  • Minimum of 2-3 years of clinical nursing experience, preferably in hospice, palliative care, or a related field.
  • Comprehensive understanding of hospice philosophy and end-of-life care principles.
  • Strong communication and interpersonal skills to effectively communicate with patients, families, and the interdisciplinary team.
  • Empathy, compassion, and sensitivity when dealing with patients and families experiencing grief and loss.
  • Ability to work independently and as part of a team, demonstrating strong organizational and time-management skills.
  • Proficiency in using electronic health records and other relevant healthcare software.
