Case Manager

The case manager is responsible for the coordination, referral and support of medical, social, and community services for participants. The central role of the case manager is to provide person-centered, individualized attention to participants in need of care coordination, referrals and assistance accessing services. The case manager manages a caseload of identified and/or assigned individuals, conducts both telephonic and community -based outreach and provides comprehensive documentation of all activities in the EMR/system of record.
Case Managers will be required to provide most services on-site at group homes or other community-based locations but will have the ability to provide some services remotely from home if a private workspace is available in order to ensure patient privacy.
Requirements:Education/Experience:
- Experience working with adult IDD (Intellectual and Developmental Disabilities) population
- Experience working with individuals with chronic illness and behavioral health issues
- Experience navigating community and state resources within Rhode Island
- Experience in person-centered care planning and working with individuals to establish and pursue goals
- Driver's License and reliable transportation
- Conflict-Free Case Management and/or Person-Centered Care training preferred
Job Skills:
- Ability to work as part of an interdisciplinary team and actively participate in team meetings and case conferences
- Excellent communication skills and the ability to interact respectfully with culturally and linguistically diverse populations
- Ability to work independently
- Ability to communicate effectively regarding self-direction (risks and responsibilities)
- Competent in demonstrating the core components of case management (information gathering, person-centered planning, connecting to services and supports, and plan monitoring with follow-up
