Position: Homeless Outreach Coordinator

Classification: Non-Exempt

Supervised by: Program Manager

Supervises: Outreach Team

Home Start, Inc. operates programs designed to prevent homelessness, interrupt the cycle of homelessness, and promote self-sufficiency. With rising rates of homelessness in San Diego exacerbated by the pandemic, our programs are designed to ensure homelessness is rare, brief, and non-recurring.

Job Summary:

With support and guidance from the Program Manager, the Outreach Coordinator manages homeless outreach activities, oversees the day-to-day operations, and coordinates outreach events with community and city partners. The Outreach Coordinator supervises, trains, and evaluates outreach staff to ensure compliance with best practices and excellent delivery of services. This position is part of Housing’s Leadership team and works closely with Program Management, Team Leads, and Executive Team. The Outreach Coordinator will coordinate with internal and external staff to ensure comprehensive services to individuals experiencing homelessness to secure stable housing and self-sufficiency.

  • Manages the street-based outreach team, coordinates outreach activities, and ensures application of evidenced-based interventions.
  • Provides field-based training to outreach staff on policies and procedures, best practices, and administrative requirements to ensure delivery of a high standard of care and services to individuals experiencing homelessness.
  • Identify community meetings and training opportunities that promote community collaboration, professional growth, and enhance service delivery.
  • Collaborate with other homeless entities for Coordinated Entry System (CES) screenings, connections, and referrals.
  • Meets with clients and assist in the screening, assessment, and care coordination of clients in need of substance, medical, psychiatric, and other services.
  • Assists team with resolving participant emergencies in the field and ensures appropriate service coordination and interim/permanent housing placements.
  • Work collaboratively with team to connect to resources, complete/collect documentation, and secure housing placements.
  • Assists with transporting clients, coordinating events, and performing other special functions (homeless count, city council meetings, community presentations).
  • Maintain and develop relationships with partner agencies and federal, state, and local funders to ensure continued funding and contract compliance.
  • Attend conferences, meetings, and training on behalf of the department and/or agency.
  • Conducts regular data and file audits to ensure adherence to contractual obligations.
  • Assists management with collection of data, submission of monthly/quarterly reports, and preparation to audits.
  • Knowledgeable about the contract, expenditure goals, and programmatic guidelines and requirements.
  • Other duties as assigned.

Qualifications:

  • Must be vaccinated from Covid-19.
  • A minimum of a bachelor’s degree or equivalent in social work, psychology, education and/or related human services field required
  • Knowledge of family systems, community resources, substance abuse treatment and interventions required
  • Must submit to a background check via Live Scan (with ongoing alerts), as required by specific programs, funding requirements and licensing regulations.
  • Must have a valid driver’s license and a satisfactory driving record.
  • Must demonstrate ability to be culturally sensitive and respect diversity.
  • Ability to work in a fast-paced environment with exceptional problem-solving skills

Physical Demands:

  • Reading and typing on a computer frequently.
  • Sitting for long periods of time at required meetings or in supervision.
  • Daily traveling in car to meet with clients and attend agency and community meetings.

Work Environment:

  • Office space may be shared.
  • The noise level fluctuates from quiet to noisy.
  • Some travel to meet clients.
