Seeking a nursing opportunity where you can make a real impact? Interim HealthCare has an immediate opening for an experienced Registered Nurse (RN) to serve as our Clinical Manager. In this role you'll be responsible for all patient care services provided by the agency. This includes making patient and personnel assignments, coordinating patient care and referrals, assuring that patient needs are continually assessed, and assuring the development, implementation, and updates of the individualized plan of care.

What you'll do as a Home Health Registered Nurse (RN) Clinical Manager:

  • Oversee ongoing interdisciplinary assessments of patients.
  • Develop, evaluate, and adjust care plans in collaboration with patients, their representatives (if applicable), and caregivers.
  • Ensure services ordered by physicians are delivered as per the care plan.
  • Counsel and educate patients and families on healthcare needs and care goals; develop educational materials to support patient progress.
  • Ensure timely and accurate clinical documentation, including patient progress towards goals.
  • Manage and coordinate patient care services, including referrals and interdisciplinary communication.
  • Assign personnel and patient cases; supervise paraprofessional and licensed staff.
  • Facilitate effective communication with the healthcare team and physicians to coordinate care and plan for discharge.
  • Participate in Quality Assessment and Performance Improvement (QAPI) programs.
  • Engage in in-service training and contribute to the Office’s Emergency Preparedness program as needed.
  • Adhere to accepted standards of nursing practice; perform assessments and procedures requiring advanced knowledge and skills.
  • Conduct home visits to evaluate care quality and review clinical notes.
  • Use clinical judgment to manage patient populations efficiently.

Requirements:

A few must-haves for our RN Clinical Managers:

  • Licensed registered nurse (RN) in South Carolina or compact multi-state license.
  • RN license is active and in good standing.
  • Graduate of an accredited school for applicable discipline.
  • Preferred: 3 years of home care experience.
  • Preferred: 2 years of management experience.

This position requires a strong background in clinical practice and management within the home care setting.
