The Provider will be responsible for providing the following Services:

1. Facilitation of the teen IOP group #5 days/week from 5-7 PM, Mon -Friday .

2. Providing the following case management services for members of the teen IOP.

a. Contacting providers outside of Exult that have either referred or treat group members for coordination of care regularly or as needed.

b. Treatment planning and discharge planning.

c. Assisting in curriculum development and review.

d. Attending treatment team weekly for case review.

3. Psychotherapy services in line with the providers licensure requirements and limitations.

a. These services are only to be provided in-person at your physical work location.

b. Telehealth may only be completed offsite if the provider is credentialed the insurance provider of the client/patient being seen via telehealth.

c. Other modalities included in this service may include family therapy, couples therapy, crisis interventions, termination sessions, and/or group therapy as the provider feels necessary for their patients/clients.

4. Assessment of bio-psycho-social needs and treatment coordination.

5. Ensuring the proper care of clients as mandated by the provider’s ethical guidelines and in coordination with other treating professionals.

6. Ensuring proper and timely documentation of services provided.

a. The provider is to use only the agency-provided tools/programs/forms for documentation unless prior approval is gained from the clinical director and/or medical director.

b. Documentation of services provided should be completed within 24 hours of the service rendered and no later than 72 hours.

c. Treatment plans must be approved by the client/patient or their guardian and must be updated every 25 days.

7. Providers providing group facilitation services must obtain coverage for group sessions before taking time away from the practice. In urgent/emergency scenarios the clinical director will be responsible for this. If a provider is unable to obtain coverage the clinical director should be asked for assistance.

8. Other administrative duties as needed and assigned by the administrative team that in-line with the provider’s knowledge, skillset, and comfort zone. These tasks will be assigned when the provider is falling short of 40 hours of service per week.

9. The provider is expected to maintain 28-32 hours of direct services to clients. This includes group facilitation, therapy sessions, intake assessments, and crisis services.

10. Exult Healthcare requests 90 days resignation notice whenever possible to ensure transfer of care of clients under the care of the provider.

Qualifications:

  • Up-to-date LPC/ LCSW/ LMFT professional therapy/counseling licensure
  • Competent and experienced at providing therapy
  • An honest and sincere respect for clients and relationships of all kinds, and a commitment to providing the best care possible
  • Cultural responsiveness; awareness of your own cultural identity and the ability to learn from and respectfully relate to other cultural backgrounds and heritages, including a willingness to engage in anti-racism work
  • Professional, ethical, and a good communicator
  • Comfortable with using technology both in and out of the therapy room
  • Skilled completing clinical documentation (e.g. case notes) in a correct and timely manner
