Hope Valley, United States Wood River Health Full time
Job DescriptionJob Description

Wood River Health is now hiring a Nurse Manager for the Hope Valley location. This position reports to the Chief Medical Officer and is a local leader for our care teams and a keeper of institutional knowledge and excellence. Responsible for the environment of care, clinical supervision, and training of peers including nurses and medical assistants, this position is an integral member of the extended care team. Shared management by nursing of patients with new or poorly controlled chronic conditions such as Diabetes, Asthma, and Hypertension is coordinated by this position for twice-annual visits with one of our staff nurses. This position is a 50% combination administrative and clinical work where the PCMH Site Lead RN ensures team-based care services are efficient, compliant, and fulfilling for all staff. Key indicators of success are successful use of protocolled ‘Clear-Triage,’ standing orders related outreach, and ensuring that patients with poorly controlled chronic conditions have a focused care plan documented. This work involves both educating within our department, and regular supervision of staff performing procedures and effective patient care workflows such as accurate pre-visit planning and rooming. Managing our nurses directly is a portion of this role, including scheduling and filling-in when necessary; medical assistants report administratively through our Lead Medical Assistants to the Chief Medical Officer, and clinically to Nurse Manager.

This position manages the environment of care, including exam room equipment, stocking, medication and vaccine inventory, and infection control/new employee screenings. This position also works clinically when needed with our front line nurses and provides direct patient care to cover vacancies and staff time off. Operational and administrative functions are completed after patient care is delivered. This position monitors productivity, initiatives, outcomes, and supervision goals with administrative assistance from the Chief Medical Officer.

The Nurse Manager essential duties include, but are not limited to:

  • Responsible for organizing and supervising the work of the Registered Nurses; direct reports are distributed across all Site Lead RNs by the Chief Medical Officer.
  • Ensures appropriate staffing levels, while minimizing overtime, in a fiscally responsible manner necessary for uninterrupted patient care during hours of operation.
  • Manages PCMH patient interventions and processes/ protocols to address patients with chronic conditions who are not otherwise engaged in a Care Management program.
  • Monitors and participates in Wood River Health processes for follow up of abnormal results.
  • For direct reports, collaborates with the Chief Medical Officer in conducting annual performance reviews and provides regular feedback to medical assistants and nurses intended to foster personal and professional growth.
  • Performs and/or participates in competency testing of staff, within training /expertise and scope of duties, in collaboration with our Clinical Care Coordinator who will track this information.
  • Works seamlessly with the Clinical Care Coordinator to ensure the completion and documentation of onboarding and annual educational requirements.
  • Responsible for modelling professional behavior and communications in the best interest of our patients, staff, and Wood River Health. This includes day to day constructive interventions when an issue is observed.
  • Document issues in staff care delivery within 24 hours of incidents or becoming aware of an issue using our incident reporting software; participates in remediation through education and recommendations for any further actions; documents the delivery of verbal warnings using a factual follow up email copied to the CMO and human resources.
  • Develops department-related, regular in-service training activities for nurses and medical assistants according to the annual schedule developed with the Chief Medical Officer.
  • Actively facilitates the regular achievement of departmental quality and productivity goals expected in the WRH strategic plan.
  • Possesses knowledge of state, federal, and OSHA regulations, and implements policies and procedures to ensure compliance with regulations and guidelines including equipment sterility, vaccine management, and point of care medication storage and administration, and point of care testing.
  • Ensures the inventory and ordering for department to ensure consistent stock of clinical supplies while minimizing waste from expiration. This may be delegated but must be monitored monthly.
  • Works with our internal and external vendors on the routine maintenance of medical and office equipment, recommending repair or replacement to the Chief Medical Officer.
  • Complies with federal, state, and health center requirements in the areas of protected health information and patient privacy.
  • Participates in committees and projects as requested by Chief Medical Officer
  • Represents Wood River at external events when needed, which could be nights or weekends
  • Maintain strict confidentiality in all matters
  • Operate within the scope of the Health Information Portability and Accountability Act to safeguard the privacy of protected patient health information
  • Enliven and support the mission, vision, and values of Wood River Health
  • Adhere to organizational policies and procedures and Wood River Health Compliance Program Standards
  • Performing other duties as assigned to meet business needs

SUPERVISORY RESPONSIBILITIES:

  • Hire, train, and supervise employees within functional areas of responsibility. Evaluate performance and regularly provide feedback.

  • Experience in process optimization and improvement a plus

  • Current CPR required, and knowledge of basic first aid
