Wood River Health has an exciting opportunity for a Nurse Care Manager in the Behavioral Health Department. This position provides support to the program and oversees, supports, and delegates care given to patients. Promotes health through education, collaboration, and outreach activities. Utilizes nursing process, provides physiological and psychological support to patients. Participates in professional development by attending programs, workshops, in-services both internal and external, as well as department meetings.

The Nurse Care Manager essential duties include, but are not limited to:

  • Provide patient-centered care within the nursing license scope of practice, including the initial assessment and intake, medication initiation, stabilization, and maintenance phases of treatment.
  • Collaborate with program partner providers, social workers/counselors, psychiatrists, pharmacies, primary care providers, recovery support persons, and specialty care providers to whom the patient has been referred both internally and externally.
  • Coordinate between the program providers and the pharmacy by assisting with prescription processing and refills, prior authorizations, and insurance issues.
  • Complete appropriate documentation in medical records and comply with the scope of practice and federal, state, and departmental policies when sharing and/or documenting patient care.
  • Must be able to prioritize and complete information requested in a timely fashion.
  • Medication administration.
  • Communicate with the patient's provider on an ongoing basis.
  • Secure guidance or help as needed with department manager and support staff.
  • Educate patients regarding treatment procedures, medications, or physicians' instructions to patients.
  • Telephone and walk-in triage of patients.
  • Participate in professional development by attending programs workshops, in-services, assigned committees, department and staff meetings.
  • Attend ongoing professional education on topics relevant to treatment for SUD (e.g., HIV and HCV screening, treatment and prevention, urine toxicology screening, overdose education, motivational interviewing, retention in care, harm reduction, compassion fatigue, case discussions, materials development, and networking).
  • Participate in community events representing Wood River Health, as appropriate.
  • Maintain strict confidentiality in all matters.
  • Operate within the scope of the Health Information Portability and Accountability Act to safeguard the privacy of protected patient health information.
  • Enliven and support the mission, vision, and values of Wood River Health.
  • Adhere to organizational policies and procedures and Wood River Health Compliance Program Standards.
  • Performing other duties as assigned to meet business needs.

EDUCATION AND EXPERIENCE:

  • Graduate of accredited nursing program, Bachelor of Science in Nursing preferred
  • Achieved or be working toward a certification in addiction nursing (Certified Addiction Registered Nurse [CARN]), offered through the Addiction Nursing Certification Board strongly preferred
  • Experience working in a community health center, preferred
  • Experience working with patients experiencing addiction
  • Ability to effectively use MS Office Suite, Internet, and email
  • Experience with electronic health records, preferred
  • Cultural sensitivity necessary to work with a diverse patient and staff population

LICENSURE AND CERTIFICATION:

  • Current, full, unrestricted RI Registered Nurse license
  • Current BLS
