Adult & Child Health is an integrated healthcare provider in Central Indiana. About this role: Our Community Health Worker helps to support our mission and team so we can work in the most efficient and effective way. You will have the opportunity for flexible scheduling. The Community Health Worker is a member of a multidisciplinary, integrated team of behavioral health and primary care providers. The role of this position within the team is to provide consumer education, crisis services and support for identifying and accessing community resources.


The Location and hours: A&C is based in Central Indiana. For this position, we service the following counties: Northwood, Marion Co. Core Business hours. Flexible scheduling


Responsibilities include but not limited to:·

  • Assist patients with their overall healthcare goals by utilizing evidenced based treatment strategies.
  • Advocate for patient needs
  • Ability to manage caseload and meet quality care standards
  • Document progress notes and patient reports accurately and timely
Requirements:

  • Completed Bachelor's degree in Social Work, Psychology, or related behavioral health field
  • Must be eligible for the Indiana Community Health Worker (CHW) Certification. For details of the certification requirements go to www.chwcrs.org to ensure you meet the eligibility. Being CHW certified is a plus, but CHW training will be provided to those hired.
  • Must hold a valid driver's license with acceptable driving record and provide own, insured, transportation
  • Ability to flex between clinic locations
