Community Health Worker - Housing

Aspire Indiana Health is a nonprofit provider of comprehensive "whole health" services including primary medical care, behavioral health, recovery services and programs addressing the social drivers of health such as housing and employment. Aspire has health centers in four Central Indiana counties serving Hoosiers of all ages and walks of life.

Position Summary

Community Health Workers (CHWs) are flexible members of a health care team who work with vulnerable populations to improve health outcomes. The goal of the CHW is to improve the health of the community by increasing health knowledge and self-sufficiency through outreach, community education, informal counseling, social support, and advocacy.

The Community Health Worker - Housing will engage clients in all areas of housing, which includes a housing plan, advocating for the clients, visiting clients in their home once they are housed and developing skills to remain housed. They will also problem solve housing related issues and utilize housing practices such as diversion, shared housing, navigation, and motivational interviewing, and provide clients with ongoing assessment, matching changing needs with appropriate services and referring to needed resources (primary care, behavioral health care, recovery servcies, SNAP, Social Security, transportation, employment, etc).

Education and Experience

* High School Diploma or equivalent required, Bachelor's Degree in social work, psychology, counseling or related human service field from a college/university accredited by the US Department of Education preferred
* One (1) year of related experience preferred

Certification

* Must be able to achieve and maintain Community Health Worker certification within 12 months of hire

* Ability to work well under pressure in a fast-paced, time sensitive environment with shifting priorities and multiple deadlines
* Must possess/maintain a valid driver's license, current automobile insurance and a driving record that meets the guidelines and requirements of the organization
* Must have reliable transportation and utilize personal vehicle to transport self/others and to attend trainings and/or meetings
