As a Diabetes Educator, you'll play a vital role in promoting effective diabetes self-management for patients and their significant others. Your expertise will empower them with the knowledge and skills needed to maintain optimal health and psychosocial adaptation.

Key Responsibilities:
  • Assess patient strengths and educational needs using available data and resources.
  • Collaborate with the healthcare team to establish and prioritize individualized plans with patients and their significant others.
  • Provide patient education experiences based on standards of practice for your discipline, ADA, AADE, and other regulatory agencies.
  • Promote safe practice of diabetes self-management related to medication administration, meal planning, exercise, blood glucose monitoring, and other aspects of care.
  • Empower patients and their significant others with knowledge and skills in self-care for optimal health and psychosocial adaptation.
  • Consult with other healthcare professionals and organizations to utilize appropriate resources that promote consistency and continuity of care of high value.

Requirements:
  • Graduate of a National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience.
  • Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC).
  • Two years or more of nursing experience required.
  • BSN required.
  • Certified as a diabetes educator preferred before hiring.
  • One to two years of educating patients in the area of diabetes preferred.
  • Excellent communication skills.
  • Excellent team skills.
  • Compassionate and caring.
  • Accountable and self-directed.
  • EPIC experience a plus.
