Case Manager- Healthy Transitions Program

The Case Manager (CM) will work as part of the Healthy Transitions team to assist youth and young adults with emotional needs and their families within the community and natural environments. The Case Manager will provide education and direct support to help the youth and their family strengthen their natural support system. The CM position is designed to provide intensive levels of support for youth and families, through assistance in accessing and linking to resources and services within the community to best meet the individualized need. The Case Manager position requires a Bachelor's Degree in social work, psychology, or a related field from an accredited college or university, as well as some knowledge of the mental health and social service field. The individual should possess good judgment skills, good communication skills, and the ability to organize their time to meet the individuals' needs in a flexible, non-traditional time frame. Able to drive unassisted and maintain a valid driver's license.

QUALIFICATIONS/EDUCATION:

The position requires a minimum age of 18 years with a bachelor's degree.

• $1500 sign-on bonus (50% granted at 6 months of employment, remainder granted at 1 year of employment.)

Salary starts at $38,000.00 per year.
