The Youth Support Partner will work as part of the Healthy Transitions team to assist youth and young adults with emotional needs and their families within the community and natural environments. The Youth Support Partner will provide education and limited direct support to help the adolescent and their family strengthen their natural support system. The Youth Support Partner (YSP) position is designed to provide intensive levels of peer support for youth and young adults, while sharing lived experience. The YSP will also support youth and their families in accessing resources and services within the community to best meet the individualized need, as well as learning the necessary skills to gain independence in preparation for adulthood. Often, but not always, the YSP is a graduate of a HFW and but shares a common lived experience as a recipient of emotional/behavioral health services and needs. The Youth Support Partner is someone who has personal experience with emotional and/or behavioral health challenges. This provides an empathetic link to the youth through which the Healthy Transitions team provides direct, non-clinical services to the youth to better ensure an individualized approach. Able to drive unassisted and maintain a valid driver's license.

QUALIFICATIONS/EDUCATION:

The position requires a minimum age of 18 years with a high school diploma or GED; have lived as a youth with an emotional disorder and willingness to disclose the shared lived experience of raising a child with emotional/behavioral needs. Requires a driver's license and access to reliable transportation.

  • Vision insurance and short-term/long-term disability insurance is offered at NO COST to the employee.
  • $1500 sign-on bonus (50% granted at 6 months of employment, remainder granted at 1 year of employment.)
  • $2500 added remuneration for bilingual ability.
  • Added remuneration per year of experience

Salary starts at $35,000.00 per year.
