We are seeking a compassionate and dedicated Individuals to join our team as an SMDI Care Coordinator.


Severe and Disabling Mental Illness (SDMI) Waiver Case Management is a program offered through AWARE for people 18 years and older who require long-term community support due to mental health symptoms.

Responsibilities:

  • Coordinate meetings with case managers, clinicians, community-based providers, and/or support workers;
  • Maintains a caseload established with the Service Director in order to work closely with programs in assigned service areas;
  • Assesses and evaluates adults to determine the appropriateness and need for case management services;
  • Assists in the person's entry into service and monitors the needed services throughout service system;
  • Provides coordination and comprehensive development of an Individual Treatment Plan (ITP) and monitors that plan as required by established rules;
  • Follows State of Montana and A.W.A.R.E., Inc. rules, policies, procedures, and guidelines as they apply to this service;
  • Assures that any person serviced by Adult Case Management is screened for and assisted in enrollment in the SDMI program;

Talents, skills, and abilities:
  • Bachelor’s Degree in Human Services or a related field.
  • Minimum of one (1) year direct work experience.
  • Valid Montana Driver’s License.
