Frontier Psychiatry aims to improve the emotional, behavioral, and physical well-being of rural communities by delivering scalable, timely, and evidence-based interventions in psychiatry and addiction medicine. Since its launch in March 2020, Frontier Psychiatry has expanded its telehealth services across the west coast with future plans to expand across additional states. Our long-term goal is to be the solution to the mental health and addiction crisis in rural America, expanding our footprint by two states per year for the next six years.

Job Summary:

The Social Worker plays a vital role in the delivery of mental health services at Frontier Psychiatry, providing direct patient care through individual therapy sessions, crisis intervention, psychoeducation, and care coordination. As a crucial member of our clinical support team, the Social Worker also plays a significant role in facilitating behavioral health services by scheduling appointments, reducing barriers to patient care, and contributing to the expansion of our service line. This role encompasses conducting thorough mental health assessments, crafting individualized treatment plans, and ensuring accurate and timely documentation of all therapy sessions and interventions. In their capacity as a care coordinator, the Social Worker handles clinical phone calls, manages patient communications, assists in developing our resource library, and offers essential support to healthcare providers.

Throughout all responsibilities, the Social Worker maintains strict adherence to ethical guidelines, legal standards, and licensing requirements, while delivering compassionate and effective patient care.

Key Responsibilities:

Therapy & Treatment Planning (75%):

  • Conduct initial mental health assessments to identify clinical needs, inform diagnosis, and develop treatment interventions.

  • Provide one-on-one therapy via remote platforms, utilizing evidence-based methods such as Motivational Interviewing, CBT, DBT, or EMDR.

  • Develop and update personalized treatment plans, ensuring they align with patient needs and therapeutic goals.

  • Document sessions and progress towards treatment goals.

  • Conduct comprehensive risk assessments and safety plans to address acute mental health or behavioral crises in collaboration with care team members.

Psychoeducation:

  • Offer educational sessions to patients on various mental health topics, including stress management, anger management, communication skills, and coping mechanisms.

Care Coordination and Departmental Support (25%):

  • Collaborate with relevant professionals and medical staff to provide Behavioral Health Integration (BHI) and Primary Care Management (PCM) interventions and care management to improve outcomes and wellbeing.

  • Responsible for addressing patient questions in a timely manner which includes answering phone calls, responding to text messages and patient portal messages.

  • Triage incoming patient correspondences appropriately (i.e. fax, email, website, text, phone, mail, etc)

  • Oversee providers schedule as required and ensure patient charts are prepared by abstracting clinical data including requesting outside records when needed.

  • Follow up on pending referrals per policy.

  • Provide outreach as necessary to patients to assist with scheduling and re-engagement.

  • Educate and discuss referral processes and expectations with patients.

Documentation and Record Keeping:

  • Maintain accurate and confidential records of each therapy session or intervention, including progress notes, treatment plans, and assessments.

  • Complete all documentation in a timely manner within the Electronic Health Record (EHR) system.

Ethics and Legal Compliance:

  • Adhere to ethical guidelines and legal requirements related to patient confidentiality, informed consent, and mandatory reporting of harm to self or others.

  • Participate in weekly supervision, rounding, or case consultation as directed by LCSW lead

Qualifications:
  • Master's degree in Social Work (MSW) or Counseling Psychology from an accredited institution.

  • Licensed as a SWLC in Montana at the time of hire.

  • Be willing to obtain licensure in additional states as required to meet clinical need.

  • Minimum of 2 years of experience in direct patient practice with individuals with significant mental health diagnoses.

  • Minimum of 2 years working in care coordination and case management in a rural setting.

  • Strong understanding of evidence-based therapeutic modalities and interventions.

  • Proficiency in utilizing remote communication platforms for therapy sessions and documentation.

Knowledge & Abilities:
  • Strong clinical assessment and diagnostic skills.

  • Excellent interpersonal skills, with the ability to communicate effectively with patients and healthcare providers.

  • Ability to multitask and prioritize tasks effectively in a fast-paced clinical environment

  • Proficient in maintaining accurate documentation and records.

  • Ability to work independently while utilizing supervision and support as necessary to meet patient needs.

  • Commitment to ongoing professional development and adherence to ethical standards.
