Position Summary

The Program Plan Coordinator is responsible for ensuring the certification of documentation of each individual’s program plan. Program plan coordinator will assist at assigned locations to ensure quality and regulatory compliance as it relates to paperwork and training. The program plan coordinator is responsible for partnering with the coordinator to ensure all filing is complete.



Position Standards

1. Ensures the completion of written program plans and related objectives.

2. Ensures goals are appropriate for person as well as meet the 12 billable service required by OPWDD

3. Responsible for ensuring monthly notes (hab checklists) are entered for assigned program sites by the 7th of the following month on the computer using the designated software program.

4. Ensure that all Hab checklist reports have been submitted by the 10th of the following month and review for accuracy ensuring they capture the individual’s response to service and a note has been completed for all individuals on the current roster.

5. Ensures communication with billing department and trouble shoots/corrects any related billing issues/documentation errors.

6. Ensures compliance with all billing, regulations, law and other requirements as it relates to paperwork.

7. Establishes a schedule with the Director/Assistant Director for monitoring and oversight of required documentation/follow-up.

8. Demonstrates good organizational and time management skills

9. Completes level one internal audits for assigned programs and ensures all follow-up is completed and documented

10. Ensure level II audit follow-up is completed and documented for all assigned programs

11. The program plan coordinator is a liaison with Abilities First providers, outside agencies and the individuals in order to ensure regulatory compliance pertaining to documentation.

12. Track, review and approve SAP, review life plans, review and approve ISP programs

13. File all required documentation in individual files at assigned program locations

14. Primary liaison with Therap related issues/concerns/changes

15. Partner with Director/Assistant Director and coordinator to assist with training, meal time observation, active engagement and community inclusion opportunities/choice to ensure they are in compliance with the HCBS setting rules

16. Partner with Assistant Director to assist with intake packet review, screening, visits, and follow-up

17. Consistently exhibits the qualities of a committed and professional employee including but not limited to: respect toward others, strive for excellence, teamwork and unity.

18. Follows all agency and departmental policies and procedures

19. Ensures timely reporting and follow-up of participant related incidents. Participates in the investigative process as directed.

20. Will incorporate core concepts of CQL’s person-centered excellence by respecting people’s concerns and responding accordingly and will use “teachable moments” to assist people in understanding and exercising their rights.

21. Fulfills mandated reporter role by interrupting and reporting suspected abuse to administration immediately.

22. Supports agency vision and mission.

23. Adheres to Code of Ethical and Legal Behavior.

24. Performs other appropriate job-related activities as requested by your supervisor or as circumstances warrant.

  2. Lift individuals by using appropriate techniques, including but not limited to usage of tools to assist, such as the Hoyer Lift.
  3. Assist individuals in ambulation by providing physical assistance, including use of a gait belt or other equipment where necessary.

  5. Have the ability to push, pull, and maneuver adults in wheelchairs.

Requirements:

1. BA/BS in a human service field with one year’s related experience or a combination of education and experience working with developmentally disabled population or any combination of education and experience which in the view of the employer qualifies the candidate for the position, which in the view of the employer qualifies the individual, may be acceptable.

2. Basic computer skills are required. Expertise in goal writing and implementation of individuals’ program plans.
