Foxdale Village, a life plan community guided by Quaker values, is a non-profit Continuing Care Retirement Community (CCRC) located in State College, PA. Foxdale was founded in 1985 by members of the State College Friends Meeting and is founded on the values of treating all with dignity and respect. Foxdale is regarded as a premier healthcare facility both locally and nationally and has been voted "Best Retirement Community in State College, PA" now for 9 years consecutively. Foxdale Village includes 57 apartments and 148 cottages and offers a range of health services with 46 beds in skilled nursing for short and long-term care, two memory care neighborhoods, and 55 beds in personal care, all with private accommodations. We currently have an opportunity for a Director of Social Services to join our team.
Director of Social Services Job Summary:
Responsible to oversee and guide the direction of the Social Services department, including the social Worker, Clinical Liaison and Transportation Assistant. Furnish professional and necessary social work services to all residents throughout the continuum of care at Foxdale Village.
This position is expected to work at least forty (40) hours a week, is required to be available for meeting with employees working various shifts, and to respond to urgent and/or emergency situations as they arise within the designated areas of oversight. This is a non-remote position that requires reliable attendance on-site.
Essential Job Duties Include:
Resident Care, Advocacy and Support:
- Advocate and demonstrate commitment to the principles of person-centered care for each resident of Foxdale Village, at all levels of the health care continuum.
- Facilitate a smooth admission and discharge process for residents coming to and leaving from Health Services at Foxdale.
- Empower each resident to continue to live as independently as possible for as long as possible and within the full continuum of residential and health care services and facilities.
- Support the resident and responsible party both practically and emotionally through the continuum of care, especially as they are moving from one level of care to another level of care.
- Establish a confidential and trusting relationship with each resident to the extent desired by the resident. This relationship will be initiated upon the entry of the resident to the Foxdale community.
- Act as the Grievance Official for the Foxdale community, by receiving grievances/complaints from residents, responsible party or staff, following up with departments for appropriate response, and ensuring resolution of the grievance within a timely manner.
- Work in a team relationship with resident, their family or responsible party and other Foxdale departments to facilitate effective communication leading to effective coordination and provision of care services. Intervene in situations involving the resident in which delicate, tactful or objective communication is necessary.
- Offer and facilitate group support meetings based on the interests, needs and requests of the residents such as Parkinson’s Support Group, Grief Support Group or Caregiver Support Group.
- Function as the Hospice Liaison for Foxdale Village.
- Aid residents with developing and implementing their advance directives for health care purposes.
- Assist family members with planning and preparing for resident memorial services when they are to be held at Foxdale Village.
Managerial Functions:
- Supervise the Social Worker, Clinical Liaison, and the Transportation Assistant, and all essential job functions. Manage performance of the department in cooperation with the Director of Health Services.
- Serve as an integral member of the interdisciplinary team of health care professionals by participating in daily clinical team meeting.
- Develop the annual Social Services departmental budget.
- Maintain and update all Social Services Departmental policies and procedures as needed.
- Manage various facility practices, including mail sort process, health care storage area, collection/updating of resident demographic data for correct entry into electronic medical record, as well as the coordination and facilitation of various monthly meetings.
Documentation and Assessment:
- Complete MDS assessments, care and support plan review, progress notes and other mood/cognitive assessments on a regular basis for residents in health care areas.
- Provide assessment, individual counseling services or referral to the appropriate providers of professional services in consultation with the individual.
- Participate in and complete system-wide audits for monthly Quality Assessment and Performance Improvement meeting.
- Complete Denial Letters for Medicare or Medicare Advantage plans as necessary.
Perform other related duties as assigned.
Maintain regular and predictable attendance in order to meet the daily needs of the residents. Attend committee and other meetings, as assigned.
Requirements:Education:
Bachelor’s Degree in Social Work or related field is required.
Experience:
One year of supervisory experience in a health care setting is preferred. Must be able to use word-processing and spreadsheet software and the Internet to complete tasks. Also, must be familiar with email. Other computer knowledge, such as use of Microsoft Office and Excel spreadsheets, is desirable.
License/Certification:
Must possess and maintain a valid PA Driver’s License and satisfactory driving record.
