PRIMARY PURPOSE:

Provide high quality assessment, intervention, and consultation within an integrated primary care team.

This Integrated Care Consultant ( ICC) will provide brief, targeted diagnostic assessment and interventions within an interdisciplinary team. Focus on providing rapid access to high quality and evidence based interventions to address a broad range of mental health and substance use concerns, in addition to treating behavioral aspects of chronic medical illness.


ESSENTIAL JOB FUNCTIONS:

Provides brief evidence-based interventions for mental health, substance abuse, chronic medical conditions and population health

Performs evaluations by interviewing patient to obtain information concerning their current problems/symptoms, level of functioning and review and administer relevant screening tools

Provides psychoeducation to patients on wellness, prevention, treatment, illness and medication adherence

Utilize the current data/clinical computer program (Electronic Health Records System) necessary to perform billing and clinical documentation, ensuring timely completion of documentation, per FoundCare policy.

Participate in community education/activities program presentations as required

Attend and actively participate in on-going improvement and utilization review activities as well as scheduled training, meetings, and clinical supervision sessions

Is available for crisis assessment and management, to include the screening of potential risk of crisis, suicide, self-harm and/or homicide. Complete the Certificate of Professional Initiating


Involuntary Examination form for any individual meeting Baker Act criteria and collaborate with individual on a clinically appropriate safety plan.

Adhere to the high standards of health care ethics and quality within this professional field, strictly following agency rules/protocols and established federal and state regulations, including but not limited to privacy, security, confidentiality, mandatory reporting of abuse and neglect.

Collaborates with primary care providers and other team members to develop comprehensive and unified treatment plans to address patient needs

Provides evidence informed interventions to assist patient and the interdisciplinary team in managing symptoms and improved functioning

Monitors symptoms and functioning, response to care plan, and need for revisions to care plan based on readiness to change, social and contextual factors, and level of engagement

Communicates and collaborates with relevant community entities serving patients and families Engages patients in services in person and via telehealth

Supports organizational mission and initiatives, including quality improvement, assurance, and patient safety and risk management processes

Other duties as assigned

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

Work in a fast-paced work environment within a community health setting serving adults and children with comprehensive person-centered care.

Past experience or training with work in an integrated primary care setting, in-person and via telehealth,

Comfortable providing treatment across the lifespan

Ability to orally communicate effectively with others without the use of an interpreter. PC Proficient (Word, Excel, Outlook, PowerPoint)

Ability to review, understand, and apply concepts presented in training programs, conferences, and/or professional literature.

Possessing experience related to delivery of services in a social service setting.

Ability to provide brief, target assessment of DSM5 diagnoses, tailor evidence-based treatments to the individual needs of the patients, collaborating with providers, and respond effectively and sensitively to a broad range of patient and family concerns.

Willingness and ability to work flexible schedule, including evenings.


PHYSICAL REQUIREMENTS:

Ability to endure short, intermittent, and/or long periods of sitting and/or standing in performance of job duties.

Accomplish job duties using various types of equipment/supplies, e.g. pens, pencils, calculators, computer keyboard, telephone, etc.

Ability to lift and carry objects weighing 25 pounds or less over distances of 100 yards.

Ability to move, stack, and store education and prevention materials and supplies in storage area, including lifting materials and placing on shelves at levels 5’ 6” and lower.

Ability to transport self to other departments, conference rooms, and on occasion to off-site locations to accomplish job responsibilities, attend meetings, workshops, seminars, etc.

MINIMUM QUALIFICATIONS

A Licensed Practitioner of the Healing Arts includes:

  1. Clinical social workers licensed in accordance with Chapter 491, F.S
  2. Mental health counselors, licensed in accordance with Chapter 491, F.S.
  3. Marriage and family therapists licensed in accordance with Chapter 491, F.S.
  4. Psychologists licensed in accordance with Chapter 490, F.S.
  5. Certified master’s level addiction professions who are certified by the Florida Certification Board Valid
