SUMMARY DESCRIPTION OF WORK

Manages mental health files of qualifying students. Provide support to families as students transition to adult services. Observe referred students in their current school setting and present findings to the Referral Leadership Team. Decision making skills and problem-solving capabilities are critical. Highly developed interpersonal skills are necessary.

Compensation: between $19.74 and $23.54 per hour (wage is determined by total years of relevant experience)
Work Schedule: 37.5 hours per week

MINIMUM QUALIFICATIONS

  • Bachelor’s Degree in social work or related field
  • Two years’ experience in an education setting, residential or day treatment preferred.
  • Minimum 21 years of age
  • Valid driver’s license/ acceptable driving record

Essential Duties and Responsibilities

  • Completes intake/orientation, assessment, and discharge process, including written reports.
  • Establishes a positive working relationship with and knowledge of school therapists, community resources, area schools and personnel.
  • Maintains confidentiality of youth and families.
  • Maintains required records as per contract, compile data spreadsheets.
  • Always displays a courteous and caring attitude to the clientele, volunteers, and visitors of the Agency.
  • Develops care management plan with assigned students that are realistic, achievable, goal oriented and focused on client needs and desires.
  • Demonstrates ability to effectively assist students in addressing barriers by utilizing community resources and health related information.
  • Coordinates services, resources, information with student, and on behalf of student when needed.
  • Provides guidance, support and crisis intervention to student and families when indicated and appropriate.
  • Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
  • Maintains regular, prompt attendance at the office on a consistent and reliable basis.
  • Abides by all specific program and Agency procedures, policies, and requirements.
  • Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
  • Creates, maintains, and shares as appropriate a dynamic self-care plan.
  • Strives to make connections between the agency and the larger community whenever possible to contribute to the agency’s ongoing fundraising and friend-raising efforts.
  • Essential functions of this job is to be performed on company physical work site
  • Performs other program related duties as assigned.
