We are seeking a compassionate and experienced Hospice Nurse Case Manager to join our team. This role focuses on community health education and provides hospice care in patients' homes and communities.

Key Responsibilities:

  • Conduct comprehensive assessments of patients and families to determine hospice needs.
  • Develop, implement, and revise personalized care plans.
  • Administer medications and treatments as prescribed.
  • Coordinate patient care as assigned, ensuring comprehensive support.
  • Maintain accurate clinical notes and communicate with physicians and health teams.
  • Educate patients and families on self-care techniques and health counseling.
  • Participate in on-call duties and ensure necessary equipment and services are available.
  • Supervise ancillary personnel and pursue professional growth through continuing education.

Qualifications:

  • Current Registered Nurse license.
  • Experience in hospice care and community health education.
  • Strong assessment, evaluation, and communication skills.
