Family Preservation Case Manager

At Kids Central we believe every child deserves to grow up in a safe and loving home that provides a path to a bright future. However sometimes children, and their families, need additional support to ensure their safety and well-being. That is where we step in.
The Family Preservation Case Manager is responsible for the provision of case management and support services for families in Circuit 5 and works in a team approach with the Family Preservation Specialist. This includes the completion of referrals to assist and stabilize the family which includes but are not limited to mental health services, psychiatric services, housing, or other community referrals. Coordination with other agencies and community services is also required. Services are provided in the home to meet the unique needs of the families served.
ESSENTIAL FUNCTIONS FOR FAMILY PRESERVATION CASE MANAGER:
- Provides direct services to children and their families which may include:
- Reviews comprehensive assessments with the family
- Reviews treatment plans and contracts for services with the family
- Establishes goal setting with families
- Manages and ameliorate crises with families
- Participates in discharge planning, which may include plans for follow up treatment and connection to community services and programs
- Provides case management services
QUALIFICATIONS:
Education/Experience
Bachelor’s Degree in the human services field or BSW required and three years’ experience in a comparable position in a human services agency preferred but not required.
Licensure/Certifications
Current Background Clearance Screening Letter
Valid Florida driver’s license
Proof of vehicle insurance
KNOWLEDGE, SKILLS & ABILITIES:
- Excellent oral and written skills and ability to communicate comfortably with children, parents, and community agencies/personnel
- Organized and able to work independently with minimal supervision.
- Flexible and able to multi-task
- Excellent working knowledge of automated systems (systems used for this position include but are not limited to Florida Safe Families Network (FSFN), Excel, Microsoft Outlook, Word)
- Ability to work well as a team member and independently
- Experience in providing case management services, coordinating systems of care, working within complex family systems and experience providing in-home services
WORK ENVIRONMENT AND PHYSICAL DEMANDS
This job regularly operates in the field (homes, community agencies) and in a professional office environment. This job consists of regularly driving your personal vehicle.
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. The employee must also frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. The employee is regularly required to bend/stoop and kneel. Repetitive motion is also required. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
