General Summary

Assist the Wellness Director of Nursing Services in providing direct nursing care during second shift. Supervises the day-to-day activities performed by the nursing department team in the absence of the Wellness Director. Such services and supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility to ensure that the highest degree of quality care is maintained at all times. Every effort has been made to identify the essential functions of this position. However, it in no way implies that the duties detailed herein are the only duties you will be required to perform.

Required Duties

Nursing Care Functions:

  • Assists in maintaining of records relative to the nursing needs, assessments, and service plans for each resident. Provides nursing department personnel, dietary and administration relevant information regarding resident service plans and the facility’s ability to meet those needs.
  • Notifies Wellness Nurse of any information pertaining to the completion of assessments and service plans according to regulatory schedule and upon change in resident condition.
  • Inform all departments of new admissions and their needs.
  • Ensure quality case management to each resident in regards to Health Services
  • Monitor medication technicians and medication passes to ensure that medications are being administered and documented as ordered.
  • Monitor CNA/RA assignments and ensure documentation is completed according to service plan specifications
  • Assist in the reconciliation of medication records at month’s end and on an as needed basis.
  • Report any medication errors to Administrator and ensure the development and implementation of corrective action.
  • Communicates with physicians and community agencies relative to resident needs, physician orders and medications.
  • Ensures that physicians’ orders are transcribed properly and communicates with pharmacy.
  • Assists in ensuring the efficiency of resident medical transportation program, ensuring that resident transportation to MD visits are scheduled and occurring.
  • Ensures inter-agency correspondence forms are completed and submitted for medical visits.
  • Assists Wellness Director in ensuring all aspects of infection control program including bio-waste clean-up and staff education.
  • Report all discrepancies noted concerning MD orders, diet changes, charting and med errors to the Wellness Director and/or Administrator
  • ON-call duties every third weekend managing issues that arise and ensuring call-outs are filled.

Administrative Functions:

  • In the absence of the Wellness Director, participate with the Administrator in facility surveys and inspections made by the Department of Health and other authorized government agencies.
  • Respond to inquiries and requests by ombudsmen, and community agencies, residents and families.
  • Make recommendations to the Wellness Director and/or Administrator in regards to resident needs and status changes. Assists the nursing staff, Administrator, resident, and family in discharge planning when necessary.
  • Attends and participates in regular Department Head and staff meetings.
Requirements:

Qualifications and Education Requirements

  • EDUCATION and/or EXPERIENCE At least two (2) years of nursing (supervisory/staff experience in Long Term Care Facility or other related health care facility. Must possess as a minimum, a Licensed Practical Nursing License from an accredited nursing program. Must hold valid, current and unencumbered Nursing License for the State of Rhode Island. Must maintain appropriate continuing education credits.
  • COMMUNICATION SKILLS / English - Ability to read, speak, analyze, and interpret
  • CERTIFICATES, LICENSES, REGISTRATIONS Must have and maintain an active licenses as stated above.
  • CRIMINAL BACKGROUND Local, state and federal Background Criminal Investigations (BCI checks), and national fingerprinting registry must validate no history of criminal activity.
  • PHYSICAL DEMANDS The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
  • While performing the duties of this job, the employee is frequently required to walk, sit, use hands or feet; and talk or hear. The employee is required to stand and reach with hands and arms. Required to push or pull cart weighing 50 to 100+ pounds. Specific vision abilities required by this job include close vision.
  • Exposed to body fluids, infection, odors, air contaminants, hazardous chemicals and behavior of residents
  • Must be in good general health and demonstrate emotional stability in order to cope with the mental and emotional stress of the position
