Family Strengthening Case Manager

JOB SUMMARY:

The Family Strengthening (Links) Case Manager engages the service consumer (client) of an agency program or service in designing and following a goal-oriented plan that will respond to their physical health, behavioral health, social and environmental needs identified through formal assessment or reassessment interviews. The case manager empowers the consumer to maximize use of personal and informal resources, links the consumer to community and formal resources as needed and assures that resources are the least restrictive and least intrusive possible. The clinical counselor/case manager acts to ensure that the client obtains timely access to needed services, appropriate levels of care and continuity of care. The role of the case manager varies on individual need of the consumer (client) as follows:

RESPONSIBILITIES

  • Provide case management services that include but are not limited to: supportive counseling; visitation services (minimum twice weekly); timely (within 24 hours) case notation of all oral and written communication; timely referrals for services including, but not limited to, domestic violence counseling, specialized mental health services, and/or substance abuse services.
  • Provide reunification services to assess the potential for parents to demonstrate a minimum standard of parenting including safety and protection, as well as rehabilitating and stabilizing family relationships
  • Conduct intakes, provide assessment to determine client needs.
  • Provide face-to-face home and community visits to assess need and assist in accessing resources (minimum twice weekly).
  • Develop a comprehensive service plan.
  • Facilitate access to additional resources as needed.
  • Utilize Motivational Interviewing strategies to resolve social and emotional barriers to reunification. and to support conditions for change.
  • Create and maintain consumer files and complete all required documentation:
    • Orderly, legible and timely records of service, case notes and other documentation as required.
    • Prepare high quality drafts of program reports, recommendations, assessments, etc.
  • Provide services outside the traditional workday based on individual consumer need.
  • Provide written or an oral report to the children and youth agency and family court as needed.
  • Uses sound judgment regarding when to request supervisory assistance. Uses supervision to identify and address professional development needs.
  • Demonstrate empathy, concern and a trauma-informed care for individuals
  • Demonstrate a sense of urgency in relation to the serious nature of the consumer’s circumstances.
  • Pursue continuing education to maintain and upgrade social service knowledge and practice skills: Participate in required training. Acquire required training hours in a timely manner.
  • Demonstrate knowledge of culturally competent values, principles and practices: Attend to on-going development of sensitivity to cultural groups, parenting practices, regional diversity, etc.
  • Understand and observe each client’s right to privacy and confidentiality as articulated by agency policy and procedures
  • Perform administrative tasks and other duties as assigned

QUALIFICATIONS:

Education: Bachelor’s degree in a closely related human services field

  1. Knowledge of child and family service systems
  2. Valid Driver’s License (issued by state in which employee lives) and safe driving record and proof of insurance coverage
  3. Knowledge of social work values and/or child welfare services
  4. Experience with computers and web-based systems

Work/Internship Experience: Two or more post-graduate years of successful employment (preferred) or internships. Two years case management experience involving child and family services (preferred)

  1. The work experience should reflect service to clients with characteristics similar to the specific population target by the LINKS program (parents with children in the custody of the county agency; work with or within county agencies).
  2. Comprehensive knowledge of community-wide health and human service systems and facility in differentiating the specific resources appropriate for the problems commonly encountered by the client population serviced in this program and position. The work experience should indicate increasing levels of responsibility, independence and proactive advocacy for and with clients.
