JOB SUMMARY
Under the supervision of the Senior Director of Integrated Services, the Manager of Peer Recovery Support Services functions as Patient Navigator for the Passaic County Opioid Overdose Recovery Program, and also supervises and provides oversight to the Recovery Community Center staff and volunteers providing recovery support services to adults afflicted with substance use and/or co-occurring disorders.
JOB DUTIES
Performs all procedures accurately, completely and efficiently for clients in accordance with applicable program policies and procedures:
- Is on-call at all times (24/7) and functions as Patient Navigator for the Passaic County Opioid Overdose Recovery Program
- As The Patient Navigator, will work with individuals to support and strengthen their capacity to achieve and maintain recovery and engage in health practices; the role of the Patient Navigator/trainer shall include, but is not limited to the following:
- Referring the client directly to detox, treatment or recovery support or utilizing the DMHAS Interim Managing Entity (IME);
- Providing a warm hand-off to appropriate treatment and recovery services - this shall routinely include referrals to DHS licensed programs that offer medication assisted treatment (MAT) as a viable treatment option;
- Developing an integrated recovery plan as identified that is consumer-centered and includes strategies for recovery;
- Linking patients with systems that provide resources, services and opportunities;
- Developing linkages to resources that address specialized needs, such as agencies providing services related to HIV/AIDS, mental health disorders, chronic and acute health problems, pregnant and parenting women, veterans, and problems stemming from involvement with the criminal justice system;
- Advocating on behalf of the patient, will provide training for hospital and program staff involved in the Opiate Overdose Prevention Program that is implemented in the hospital;
- Providing assistance to the Recovery Specialist as needed; and
- Updating and monitoring the electronic on-call schedule for Recovery Specialists.
- Coordinates, develops, monitors, and supervises the activities of subordinates.
- Coordinates and supervises Recovery Specialists and Case Managers for the MWRAP program and linked women’s services
- Develops and recommends new or revised program goals and objectives for the Recovery Community Center
- Develops and schedules program work plan in accordance with specifications and funding limitations
- Oversees daily operations and coordinates activities of program and determines priorities
- Monitors program expenditures ensuring that budget allocations are met
- Assists Peer Advisory Board in preparation of proposals for funding and/or funding continuation from outside sponsors
- Coordinates activities of the Recovery Community Center with inter-related activities of other programs, departments or staff to ensure optimum efficiency and compliance with appropriate policies, procedures and specifications
- Prepares periodic reports, financial statements and reports on program activities, progress, status or other special reports for management or outside agencies
- Evaluates program effectiveness to develop improved methods; devises evaluation tactics and implements; analyzes results and recommends and/or takes appropriate action
- Reviews applications or other program documents independently or in conjunction with supervisor to determine acceptance or make decisions pertaining to program
- Recruits program participants, members and volunteers utilizing most appropriate promotional or marketing methods, such as flyers, brochures, new letters, activities, trainings and presentations
- Develops and writes communication and promotional literature for distribution such as newsletters, brochures and flyers; coordinates process from development through printing and distribution
- Develops and facilitates workshops, meetings or conferences; coordinates logistics, scheduling and participant communication
- Interacts and maintains liaisons with participants, volunteers, board members, staff, and outside/community agencies in facilitating program objectives
- Coordinates and supervises program outreach
- Supervises distribution and collection of participant surveys for quality assurance
- Conducts biweekly recovery specialist trainings
- Any projects/activities as deemed necessary by the Executive VP-Clinical Services.
QUALIFICATIONS
EDUCATION and CERTIFICATION:
Must possess a bachelor’s degree in health, psychology, counseling, social work, education or other behavioral health profession or two years of full-time equivalent experience in the substance-related disorders field.
Must possess a Certified Peer Recovery Specialist certification (CPRS) and under the supervision of a person who has the required credentials and at least three years’ experience in the substance-related disorders field.
EXPERIENCE:
Knowledge of all aspects of addiction treatment and how to access it, including the Stages of Change Theory; Motivational Interviewing; behavioral problem solving techniques; principles of recovery for either substance use or mental health disorders; case management activities and applications particular to recovery management; indigenous and community competencies; community resources for social support and how to access each; resources for food, clothing, shelter and other distal needs and how to access; resources for medical care and how to access; resources for mental health care and how to access; mutual aid recovery groups, their functions, values and beliefs and how to access; faith-based organizations and how to access; confidentiality rules and regulations related to substance use and mental health disorders prevention, treatment and recovery supports.
The Patient Navigator (OORP) must possess the knowledge, skills and experience necessary to competently perform case management activities. The Patient Navigator must have at least three (3) years’ demonstrated experience working with high need individuals with substance use and mental health disorders. The Patient Navigator shall demonstrate evidence of addiction coursework. The Patient Navigator will work with individuals to support and strengthen their capacity to achieve and maintain recovery and engage in health practices.
Must be nonjudgmental of adolescents or adults who have a substance use or mental health disorder.
Must have ability to establish empathy with the individual.
Must have ability to work with diverse populations and cultural backgrounds.
Must be comfortable in working independently in community settings.
Must have willingness and open-mindedness to follow established procedures.
Must have acceptance of extremely flexible working hours.
