What is a Care Manager?
Care Managers are non-clinical team members who are skilled at engaging with patients over the phone and in person. Care
managers work with clinical care teams to enhance patient-centered care. A care manager helps patients navigate the complex
healthcare system and act as a liaison or an advocate for vulnerable populations (uninsured, homeless, etc.). Care managers function
as members of our health centers care coordination team to perform tasks such as patient engagement and outreach, connecting
patients to patient-centered care, improving clinic quality measures by providing resources internally and externally, and
demonstrating professionalism. The purpose of the care manager is to serve as a link between the individual and community
resources with the goal to:
1. Identify a medical home where a person has access to consistent healthcare
2. Support the low-income community in accessing appropriate health care services and resources.
3. Assist patients in removing non-medical barriers to improving healthier outcomes.


Job Duties/Responsibilities
• Accurately describes care management services to patients and encourages their participation.
• Participates in outreach events to raise community awareness for new & existing patients.
• Meets with patients in person and on the phone to assist with gaining access to community resources: education,
patient assistance, transportation, insurance, etc., where applicable.
• Linkage to care for specialty disease sets, including infectious diseases.
• Evaluates the effectiveness of resources/programs for the patient’s benefit.
• Schedule and confirm appointments; collect patient data needed for appointments.
• Assist patients with Market Place (ACA), Medicaid, and Medicare insurance enrollment
• Document all patient interactions and encounters in the EMR.
• Assist with patient education on chronic diseases in person and by phone.
• Enhance patient connection to PCMH (Patient-centered medical home)
• Effectively able to implement crisis intervention and problem-solving skills as needed.
• Ability to work both independently and within a team environment.
• Additional duties as assigned.


Minimum Qualifications
• Ability to pass a background check
• An associate degree, preferably a bachelor’s in a health-related field
• Ability to successfully pass a background check and pre-employment physical health exam.
• At least one year of experience working in the healthcare field.
• Proficient in Microsoft software (Word, Excel, PowerPoint, Teams, Outlook, etc.), windows-based computer applications,
• and online research.
• Excellent oral and written communication skills and the ability to carry out written and oral instructions.
• Understanding of and the ability to work with current communities or demographics served by the health centers.
• Detail-oriented, organized, and flexible
