This position is tasked with overseeing the daily functions of the care management program, which encompasses various essential components including:
- Care facilitation
- Utilization management
- Discharge planning
- Transitions of care

The incumbent will play a pivotal role in developing and refining systems and processes related to care and utilization management within the healthcare facility. Additionally, this role involves:

  • Managing the department's daily activities associated with discharge planning and clinical quality enhancement.
  • Evaluating the appropriate and effective use of hospital resources.
  • Assisting in the collection, analysis, and reporting of financial and quality metrics pertinent to utilization management.
  • Leading and motivating staff to adapt and thrive in a dynamic healthcare environment.
  • Promoting interdisciplinary collaboration and fostering a culture of teamwork and service excellence.

Minimum Qualifications:
Education and Licensure:
- Bachelor’s degree in Social Work.
- Licensed Social Worker (LSW) through the relevant board.

Experience:
- A minimum of five years of social work experience, including two years in a supervisory capacity.

Preferred Qualifications:
Education and Licensure:
- Master’s degree.
- Licensed as an LGSW or LCSW.

Experience:
- Familiarity with InterQual and MCG criteria.

Core Duties:

  • Overseeing all aspects of care management operations.
  • Ensuring the availability of a qualified and cohesive workforce to meet patient care demands.
  • Advising on staffing decisions, including hiring and retention.

  • Monitoring the operational budget and assisting in its preparation.
  • Facilitating effective communication among medical staff, patients, and other departments.
  • Participating in policy development and ensuring compliance.
  • Engaging in self-development activities to enhance management skills.

Physical and Working Environment:
The role may require prolonged periods of standing or sitting, with flexible hours including weekends and holidays. Participation in a rotational call schedule may also be necessary.
