Hours: 8:00 - 5:00 Monday - Friday, 40 hours per week

The Hepatitis C Bridge Coordinator is a regionally based, specialized care coordinator that will provide care referrals for people living with Hepatis C (HCV). The PC will assist clients living with HCV locate resources for payment, laboratory services, medications, and patient assistance programs for Hepatitis C cure medications. The PC is regionally based at Cabarrus Health Alliance but will have the regional responsibility for linkages to care, care management, HCV testing, and patient education for Stanly, Union, Rowan, Mecklenburg, Gaston, Cleveland, Lincoln, Catawba, Alexander, and Iredell counties.

Primary Responsibilities:

• Serves as coordinator to link persons infected with Hepatitis C to a clinical provider for care and treatment
• Facilitate rapid disease intervention by quickly engaging patients and serving as the first point of contact for persons who are newly diagnosed or not linked to care
• Provide disease specific education and guidance to clients
• Provide on-site Hepatitis C testing in collaboration with various community partners and local health departments in the region
• Conduct contact investigations for acute hepatitis C utilizing the state surveillance system

Education/Experience: Bilingual in English and Spanish preferred. High School or equivalent required. Experience in peer support, human services, communicable disease, harm reduction and/or care management required. This position will require a phlebotomy certification that can be obtained and provided after hired.

License: Valid NC driver's license

Knowledge, Skills & Abilities:

• Thorough knowledge of and skill in the application of theory, practices, principles, and techniques employed in the field of public health and harm reduction
• Considerable knowledge of HCV disease and treatment options, skilled in care management of HCV
• Ability to engage with a diverse population including those with an active substance use disorder
• Considerable knowledge of available community resources and organizations
• Ability to plan and work collaboratively with community partners and patients regionally in person and virtually
• Must be comfortable discussing social, sexual, substance-related, and other harm reduction issues and experiences in a non-judgmental way
