MECA Therapies, LLC is looking for a Care Coordinator/Community Health Worker (CHW) to join our professional team.

MECA Therapies Care Coordinator / Community Health Worker (CHW)

 

ESSENTIAL DUTIES:

  • Engaging Perspective MECA Therapies Clients including but not limited to:
    • Referral coordination  
    • Patient education
    • Client & family onboarding
    • Coordinating assessments 
  • Engaging Clients in Ongoing Services 
  • Collaborate on CHW Program Design 

In addition to the above, you will:

  • Operate within local, state, and federal regulations, as well as company policies and procedures.
  • Operate within the New Mexico Department of Health Community Health Worker Scope of Work

GENERAL REQUIREMENTS:

  • You’ll need to demonstrate excellent communication and interpersonal skills with a dedication to family-centered care.
  • You’ll need to demonstrate the ability to collaborate and work effectively cross departmentally 
  • Candidates must have a bachelor’s degree in order to apply.  In addition, you’ll need to hold or be in the process of obtaining your CHW certification through the state of New Mexico.
