Your day to day as a UAS RN AssessorJoin our team at VillageCare as a Full Time UAS RN Assessor where you will play a crucial role in conducting comprehensive assessments for potential enrollees in our Medicare and Medicaid programs. As a Nurse Assessor, you will utilize your clinical expertise to evaluate individuals' long-term community-based needs, determine their eligibility based on clinical criteria, and provide necessary training and education on self-care techniques. Conducting face-to-face UAS-NY assessments in adherence to state guidelines, you will assess various aspects of the individual's condition and communicate effectively with all parties involved. By developing comprehensive care plans that align with medical, psycho-social, and financial requirements, you will ensure the safety and well-being of our community members. This role offers the opportunity to make a meaningful impact and uphold regulatory standards while collaborating with a dedicated team focused on delivering excellence in healthcare services.
What matters mostTo excel in the role of Full Time UAS RN Assessor at VillageCare, candidates must possess an ASN degree and hold an active NYS RN License. A minimum of 2-3 years of clinical experience in home care or insurance plans is required. Proficiency in utilizing UAS-NY required and a minimum of 1 year of field-based case management experience will be highly beneficial. Strong analytical and problem-solving skills are essential, along with the ability to travel within a designated geographic area for in-person case management activities. Proficiency in standard corporate software applications such as MS Word, Excel, Outlook, and PowerPoint is necessary, along with the ability to work independently. Experience with MLTC, Managed Medicaid, MAP, or FIDA is desirable, and proficiency in languages such as Spanish, Cantonese, Mandarin, Russian, and/or Creole is a plus.
