Department: Behavioral Health & Wellness Services

Supervisor: Senior Program Manager for School Centers for Wellness

Location: Prince George's Public Schools (In-Person)

Classification: Full-time This is a non-Exempt/Hourly position

Synopsis: Under the direction of the Senior Program Manager for Schools Centers for Wellness, the School-Based Program Coordinator will support the daily services within the school-based health-based program and the related grants, including but not limited to program implementation, delivery, tracking, and data collection at school(s) assigned. The School-based Program Coordinator will develop relationships with youth, families, partners, community organizations, site staff, and school staff. This role will step in on behalf of the Senior Program Manager to facilitate and coordinate program operations when the Senior Program Manager is unavailable. The school-based program coordinator will support effective communication between school staff and LCDP staff and within the agency departments by providing clear workflows, protocols, and training for the program staff. Works directly with the SBH Senior Manager to design, develop, coordinate, and facilitate the execution of specified programs and initiatives and ongoing support of established systems and programs. Support continual improvement efforts in staff engagement, patient experience, and meeting quality metrics while implementing and sustaining quality improvement programs.

Qualifications: Required Education and Experience:

  • Bachelor's degree
  • Bilingual In English & Spanish, fluent in written and oral communication.
  • Experience developing and implementing protocols and procedures.
  • Experience working with patients and staff from diverse socio-economic, ethnic, and cultural backgrounds.
  • Ability to work independently and collaboratively.
  • Ability to work in person in and outside of a school setting.
  • Knowledge of the Latino immigrant community in the DC Metro Area and ability to work with all community segments.
  • Experience organizing and facilitating group activities.
  • Experience working with Limited English Proficient Latinos at risk for and/or the LGBTQ community.

Required Skills/Abilities/Certifications/Licenses:

  • Strong ethical core.
  • Strong leadership, interpersonal, and problem-solving skills; detail-oriented, teamwork-oriented, self-starter, and highly organized.
  • Computer literate in Microsoft Office (Word, PowerPoint, Outlook, Excel).
  • High motivation to work in the school-based mental health field.
  • Knowledge about the need for mental health and wrap-around services in the school system.
  • Excellent organizational, interpersonal, and communication skills.
  • Ability to work with individuals from various racial, cultural, and socio-economic backgrounds.
  • Ability to work with a team of staff.
  • Understanding of HIPAA (Health Insurance Portability and Accountability)
  • Comfortable working with youth and their families.
  • Knowledge of the Latino immigrant community in PGCPS and ability to work with all community segments.

Preferred Education and Experience:

  • Bachelor's degree in public health or health administration, or bachelor's degree in the human services field.
  • At least two years of program coordination experience, including developing and executing work plans, data collection, writing reports, and identifying and adapting educational and outreach materials.
  • Knowledge of positive adult and youth development curriculums.
  • Ability to build and maintain network systems within a school setting and community organizations.
  • Ability to work with electronic medical records (EMR), BH Works, and other database management and software.
  • Knowledge of Federally Qualified Health Center (FQHC) compliance.
  • Knowledge and understanding of patient/participant confidentiality issues, HIPAA (Health Insurance Portability and Accountability Act), and FERPA (Family Educational Rights and Privacy Act) compliance regulations.

Duties and Responsibilities:

  • Responsible for coordinating services at the School Centers for Wellness Programs such as Mi Refugio and School Wellness.
  • This position will perform as a skilled professional and resource in conflict resolution, team building, and engagement and will collaborate with team members, school staff, partners, and families.
  • In collaboration with the Senior Program Manager, the school-based program coordinator will be responsible for developing and implementing health, mental health, and school-based initiatives dedicated to improving the well-being of the students, family, and community.
  • In collaboration with the senior program manager, develop and implement community health initiatives to improve access to community resources that promote the students' and their families' overall health and well-being.
  • Organizes, coordinates, and facilitates various meetings, workshops, presentations, group interventions, activities, and events for parents, students, and school staff to develop partnerships between families and schools.
  • Responsible for managing the program enrollment process in collaboration with the therapist, family support worker, youth development workers, and school staff.
  • Build a network with colleges, universities, and trade schools and look for scholarships for program participants.
  • Coordinates and administers policies and procedures related to school-based mental health program enrollment, consent, and release during school activities, client visits, health education, and group activities in collaboration with the Senior Program Manager.
  • Develop and maintain strong relationships with multiple stakeholders, including school staff, program participants, providers, community agencies, government entities, community members, and others.
  • Collect and track weekly and monthly data in collaboration with the Senior Program Manager.
  • Preparing reports on program progress and outcomes and helping prepare other reports required for all program grants and/or as needed for Mi Refugio and School Wellness Programs.
  • Create and deliver a program evaluation plan to measure program outcomes in collaboration with the senior program manager.
  • Collaborate with Senior Program Manager to develop and track the workflow of Mi Refugio and School Wellness Programs.
  • Plan and conduct training to program new interns and staff.
  • Responsible for ordering office and program supplies.
  • Represent LCDP to the public, official organizations, and other groups by attending meetings, community-planning groups, etc., as designated by the program senior manager.
  • Collaborate with other program coordinators and school centers for wellness staff.
  • Collaborate and serve as a liaison between the School Centers for Wellness Program and other departments and programs of La Clinica del Pueblo.
  • Assist with the development and implementation of work plans to achieve the goals of any grant/contract.
  • Attend required training to increase/maintain skills.
  • Attend program, department, and all staff meetings and all mandatory La Clinica activities.
  • Follow La Clinica's Code of Conduct and all other personnel Policies and Procedures.
  • Perform other duties as assigned.

Supervisory Responsibilities:

  • None.

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift to 30 pounds at times.
  • Must be able to work outside and stand up for a prolonged period during outreach events.
  • Ability to work weekends and evenings as needed.
  • Able to perform duties in different La Clinica locations as required.
  • Able to perform duties and responsibilities remotely when needed.
