Position Details:We are seeking a compassionate and detail-oriented Case Manager to join our team. In this role, you will be instrumental in helping clients navigate essential services and resources, ensuring they receive the support they need to achieve their goals.


Full-time Benefit Eligible

Schedule: Monday- Friday 9 am-6 pm

Compensation: Starts at $18.00/ Hour

Willamette Family Employment Requirements:

  • Criminal Background check conducted by the Department of Human Services Background Check Unit
  • Pre-Employment Drug Screening (Including THC)

Case Manager Qualifications:

  • HS Diploma or equivilant

Skills and Experience:

  • Experience in case management or a similar role is highly desirable.
  • Strong organizational skills and attention to detail.
  • Excellent communication and interpersonal skills.
  • Ability to manage multiple tasks and prioritize effectively.
  • Proficiency in using various computer systems and software applications.
  • Knowledge of TANF, SNAP, MAT, and other relevant programs and services is a plus.

Primary Responsibilities:

  • Processing TANF/SNAP Applications and Managing Benefits:
    • Process applications for TANF (Temporary Assistance for Needy Families) and SNAP (Supplemental Nutrition Assistance Program).
    • Manage and administer Electronic Benefits Transfer (EBT) cards for clients.
  • Client Chore Program Oversight:
    • Supervise and manage the client chore program, ensuring that clients complete assigned tasks effectively.
  • Family Visitation Coordination:
    • Coordinate and oversee family visitations, ensuring adherence to all relevant protocols and procedures.
  • Community Group Documentation:
    • Document weekly community group activities and interactions for accurate record-keeping and reporting.
  • MAT Appointment Coordination:
    • Assist clients in coordinating and scheduling Medication-Assisted Treatment (MAT) appointments.
  • UA Results Tracking:
    • Track and report urine analysis (UA) results to the counseling team for appropriate follow-up actions.
  • Transportation Assistance:
    • Help clients access their Ridesource benefits and arrange transportation for necessary appointments.
  • OHP Assistor:
    • Serve as an Oregon Health Plan (OHP) assistor, guiding clients through the process of accessing healthcare coverage.
  • Care Team Meeting Scheduling:
    • Schedule and organize care team meetings to ensure comprehensive planning and coordination of client care.
  • Release of Information (ROI) Management:
    • Complete Release of Information (ROI) forms with clients to facilitate communication with other service providers as needed.
  • Care Coordination for Clients Leaving Against Staff Advice:
    • Provide ongoing care coordination and support for clients who choose to leave against staff recommendations, ensuring continuity of care.
  • Data Entry and Fidelity Monitoring:
    • Enter data into the Change Company platform to maintain fidelity monitoring and ensure accurate and up-to-date records.
