JOB SUMMARY:

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Work primarily with children and adolescents with serious mental illness or emotional disorders and their families, with the goal to maintain the youth in the least restrictive setting with regards to home, school and community programs. Work closely with youth and their families in providing the support, training and assistance required for a stable and healthy family and community life.

QUALIFICATIONS:

  • Bachelor’s Degree in Human Services field from an accredited college/university.
  • One to two years experience in the behavioral health field.
  • Experience working with children and adolescents preferred.
  • Use of own personal vehicle and valid driver’s license required.

SPECIFIC DUTIES:

  • Provide a comprehensive strengths based assessment for each assigned child/youth
  • Develop with each child/youth and family, a unified service plan addressing the needs for treatment, skill development, social and emotional development and social service needs
  • Ensure that the principles developed by Pennsylvania Child and Adolescent Service System Program (CASSP) are followed in providing services for youth and their families
  • Coordinate psychiatric services as indicated
