Reports to: Emergency Shelters Program Manager
Job Description
The Youth Emergency Shelter Case Manager helps support a wide spectrum of needs for youth (ages 13-17) experiencing homelessness who enroll in shelter. The role helps support a wide spectrum of needs for youth (ages 13-17) experiencing homelessness who are enrolled in shelter, with the ultimate goal of positively and permanently exiting young people out of homelessness and into safe and stable housing. Education and employment pursuits are integral to this work but are secondary. The role firmly understands that the success of the youth emergency shelter hinges on the complete immersion of each youth in the process, ensuring their active involvement in decision-making and goal setting, specifically the reunification of youth and family. Youth will have access to their own, individualized community support team (CST)
Duties and Responsibilities:
Manage Hope Center waitlist to ensure youth (ages 13-17) waiting are communicated in a timely manner
Conduct intake interviews for new youth, screening for program eligibility
Complete new resident intakes for all youth entering the Hope Center
Collaborate with Behavioral Health Specialist to ensure all youth needs are accounted for
Assist each youth in working through their care plan while staying at the Hope Center
Facilitate recreational, vocational, and life/job skill-training activities for youth
Maintain weekly case management meetings with each youth for regular goal review sessions to track progress, make necessary adjustments, and accommodate evolving needs
● Update Coordinated Entry housing assessments for each youth to ensure they are active in HMIS
● Build relationships with community agencies and other Case Managers to maximize referral resources to assist residents in exiting into safe and stable housing
● Maintain accurate and quality client documentation in HMIS, Infonet, and any other database Rod’s House uses.
● Collaborate with all Hope Center staff to provide youth-centered skills-building and support services
● Perform outreach, as needed, in the community to increase awareness of Rod’s House programs and services
● Submit End of Month reports to supervisor
● Assist youth with routines and support them in following shelter procedures
● Help youth work through conflict with others in shelter
● Maintain accurate and quality documentation in Rod’s House databases
● Create performance reports, as needed
● Facilitate and participate in program, department, and agency meetings
● Facilitate and participate in relevant trainings
● Facilitate and participate in local and state interagency work groups, as needed
Proven ability to create a system or process when one does not exist.
Complete all duties as assigned and other duties as required.
Required or Preferred Education and/or Experience
● One of the following Required:
o A master’s degree in social services or a closely related field from an accredited school and one year of experience working with youth or young adults OR;
o A bachelor’s degree in social services or a closely related field from an accredited school and tw0 years of experience working with children or youth OR;
o Five years of successful full-time experience in a relevant field working with children or youth; and supervisory abilities that promote effective staff performance; and relevant experience, training, and demonstrated skills in each area that they will be managing or supervising.
● Bilingual (Spanish English) Preferred

Knowledge, Skills, and Abilities
● Must be at least 21 years of age Required
Experience working with individuals who have experienced trauma, specifically severe mental illnesses and substance use disorders
● Lived experience relevant to the population served
● Experience working with young people experiencing homelessness
Salary/Compensation and Benefits
Starting wage of $20.00 per hour.

Work Conditions and Physical Requirements
Shifts could include evening, early mornings, or weekends, including holidays. This position requires interactions with young people experiencing homelessness and with community members, social service providers, and other organizations.. Physical requirements for the position include frequent walking, hearing, seeing, speaking, stooping, kneeling, crouching, reaching, grasping, climbing, and repetitive motions. This position could require exerting up to 50 pounds of force to lift, carry, pull, or move objects. Ability to safely drive company cars and vans are strongly preferred.
