Job DescriptionJob Description

The Network Development and Growth Specialist role will work for the Social Care Network in the Southern Tier region of New York and will be responsible for facilitating the onboarding process for organizations joining the Social Care Network and ensuring network adequacy for services provided by the Social Care Network. This position reports to the Network Operations Manager.

Essential Functions:

  • Identify, recruit, and support the contracting and onboarding process with Social Care Network organizations to ensure beneficiaries have access to a comprehensive and high-quality provider network.
  • Create and deploy a network adequacy assessment tool to CBOs in the SCN initially and bi-annually thereafter. Review data received to identify areas not meeting adequacy requirements (e.g., gaps in service or access) and develop and execute a plan to address these issues.
  • Develop, facilitate, and maintain the onboarding process for new social care network organizations, with duties including but not limited to: acting as the initial point of provider engagement to ensure that newly onboarded providers are adequately informed about the SCN and able to provide services to members in compliance with the provider contract; and conducting orientations to the SCN for newly contracted providers and their staff.
  • Operates as the backup to the Community Support Specialist for the SCN as the primary point of contact for all matters related to partner relations including handling communication, requests and concerns.
  • Validate CBO network participation requirements to provide and deliver services in the social care network.
  • Conducts thorough market research and analysis to identify gaps in the provider network and opportunities for network expansion.
  • Maintains a thorough understanding of social care network services and benefits and is effectively able to communicate these to prospective social care network organizations.
  • Evaluates network requirements related to members’ realized access to ensure the social care network services are meeting members’ needs and are accessible.

MINIMUM REQUIREMENTS:

  • Education: Bachelor’s degree required or 5 years equivalent experience managing and monitoring organization/partner relationships.
  • Conduct provider outreach in a positive and professional manner that builds relationships and ability to take initiative to follow up with prospective providers as appropriate
  • Excellent organizational skills and demonstrated expertise in managing relationships.
  • Possesses strong communication skills both written and verbal.
  • Exceptional ability to build and maintain collaborative working relationships with internal and external stakeholders.
  • Exceptional customer service and organizational skills, and an ability to prioritize.
  • Proficient in planning & time management
  • Ability to work independently with minimal supervision
  • Utilizes critical thinking skills
  • Skilled with personal computer programs such as Microsoft Outlook, Microsoft Office (Word, Excel PowerPoint, etc.)
  • Facilitation of meetings either in person or via virtual platforms such as Zoom

WORK ENVIRONMENT:

Care Compass Network utilizes a hybrid work model where employees are able to work remotely from a home office or from the CCN office, a non-clinical professional office setting. There may be some travel required for partner and/or network meetings.
