Medication Assisted Treatment

Summary:
The Medication Assisted Treatment (MAT) Case Manager performs a variety of case management activities to ensure that clients receive the appropriate services and to make patients aware of all the services available to them. The MAT Case Manager is the initial point of contact for any patient referrals, either personal or from other referring organizations and carries out any necessary assessments for intake. As a member of the MAT Team (which includes the Program Director, MAT Nurse, and MAT Provider), s/he is responsible for the case management of MAT patients who may need complex care coordination not only in substance use and recovery but in other domains and disciplines such as medical, dental, mental health, eligibility services, and ER visits. This position entails collecting and organizing necessary patient data (appointments, attendance, prescription dosage/refills, CURES reports, etc.). The MAT Case Manager is responsible for carrying out the mission and values of harm reduction and ensuring that all patients feel safe, are heard, and receive individualized care.
Essential Duties and Responsibilities:
- Conducts intake assessments for new and returning MAT patients (completes patient registrations, signs documents and consent forms, inputs virtual visit, etc.)
- Ensures patients are eligible for the MAT program and coordinates eligibility with other staff members, Partnership Health Plan, and other referring facilities to obtain a Referral Authorization Form if necessary
- Coordinates scheduling for all MAT patients (weekly appointments, MAT Physicals)
- Primary point of contact for local organizations and community partners referring MAT services
- Establishes group and individual counseling for MAT patients
- Assist and facilitate in group counseling both through Zoom and in-person
- Works with local businesses in related areas to provide case management for patients in need of housing, jobs, food, transportation, etc.
- Maintain a maximum caseload of 45 patients
- Coordinate meetings for the MAT team and other community partners
- Community outreach liaison attending community events to educate the local community about the MAT program and other resources and services within MCHWC
- Coordinate with other departments and staff members within the clinic to help increase the quality of life for all patients
- Effectively communicate with MAT teams members with regards to patient care and coordination
- Participate in presentations to promote the MAT program and to convey the mission of the program
- Works with patients and providers to facilitate referrals, monitor discharge planning, assist as appropriate with hospital admission and track patients in and out of the clinic environment in order to promote engagement in their care.

Qualification Requirements:
Education and/or Experience:
- Bachelor of Science Degree or equivalent work experience.
- Strong communication (oral and written) and research skills.
- Proficiency in English; Spanish desirable.
- Valid CA drivers’ license; ability to travel regularly.
- Computer experience with proficiency in Word, Excel, PowerPoint, Outlook, Zoom.
- Meet deadlines, complete projects and reports as assigned.
- Ability to work with staff, management, community organizations and agencies
- Must have a positive attitude, be a team player, and be able to take directions from supervisor
- Understand and promote the mission, vision, and values of the health center both in the workplace and in the community
- Sensitivity to the needs and situation of multi-cultural population from a variety of income levels
- Ability to manage multiple tasks
Language Skill:
- Must possess excellent organization, writing, and verbal skills
- English proficiency required; Spanish proficiency preferred
Reasoning ability:
- Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Equipment/Machinery: Fax machine, copier, personal computer, telephone, calculator, Microsoft Word/Excel, EHR system, and other software as required.

- Stand and walk or sit alternatively depending on specific needs of day. Estimate 50% of time is spent on feet and 50% sitting at desk.
- Have occasional need to perform the following physical activities: bend/stoop/squat, climb stairs, push or pull, reach above shoulders.
- Have occasional need to perform standing and walking activities.
- Constant need to perform the following physical activities: writing/typing, grasping/turning, finger dexterity.
- Lifting/carrying over 10 pounds occasionally. Lifting/carrying less than 10 pounds frequently.
- Vision requirements: constant need to complete forms, read reports, view computer screen. Frequent need to see small detail. Frequent need to see things clearly beyond arm's reach.
- Hearing requirements: constant need to communicate over telephone and in person.
Travel Requirements: Frequent need to utilize personal transportation to attend meetings, health fairs, and other events.
