Per Diem LVN

Responsibilities and Duties for LVN Hospice:

  • Perform skilled nursing tasks and home visits at patients' homes under the supervision of Registered Nurse
  • Travel to patients' homes
  • Prepare clinical notes, orders and charting in a timely manner
  • Collaborate with physicians and agency staff to deliver the best care possible
  • Work with patient and family in the home to achieve improvement in health goals
  • Manage patients health care episode with the agency
  • 1 Year experience in Hospice
  • Reliable transportation
  • Valid Driver's license
  • LVN License
  • On call duties after office hours (6pm) during weekdays and/or on weekends is optional
